本研究の目的は限局性膵萎縮（膵くびれ所見）と膵癌発症との関連性を検討し、膵癌の早期診断を目指すことである。昨年度報告したとおり、2021年7月8日に倫理委員会の承認を受けて多施設共同前向き観察研究が開始された。発足当初は3施設のみで開始されたが、2023年4月時点では全国41施設に拡大し前向き観察研究が進行中である（更に6施設を追加予定で倫理委員会へ変更申請中）。予定症例数は150例を予定していたが、施設増加のため200例に変更申請中である。現在登録症例数は53症例となっており、近日共同研究機関にCRF提出を依頼する予定である。登録症例では既に複数例の上皮内癌を含む膵癌患者が発見されており、同所見が膵癌早期診断に寄与するものと期待している。引き続きの症例登録、経過観察を継続予定である。当科で保有するデータベースでは約20年の経過で、10例の膵上皮内癌（Stage 0）が発見されている。当院は東北地方の機関病院であり、膵癌患者も多数受診するが、この程度しか発見されておらず、膵上皮内癌発見は困難を極めていた。しかし、本臨床研究が開始されてから、本学症例に限定しても既に3例の上皮内癌が確認されており、この研究に手ごたえを感じている。　限局性膵萎縮は現在注目されており、日本消化器病学会、日本消化器内視鏡学会、膵臓学系で学会発表を行った。本臨床研究は4年後まで継続するが、今後中間発表を予定している。

The aim of this study is to investigate the association between local atrophy and cancer development, and to provide guidance for early diagnosis of cancer. On July 8, 2021, the Ethics Committee acknowledged the initiation of a multi-agency joint review study. Since the beginning of the project, the project has been under way. As of April 2023, 41 projects in China are under investigation (6 projects are under review). The number of scheduled cases is 150, and the number of scheduled cases is 200. The current number of registered cases is 53 cases. Recently, the CRF has been proposed by the joint research organization. There are several cases of intraepithelial carcinoma including breast cancer, and the same findings are expected for early diagnosis of breast cancer. For example, if you want to register, you can check your profile. During the past 20 years, 10 cases of intraepithelial carcinoma (Stage 0) developed. Most of the cancer patients were diagnosed at the Tohoku Regional Hospital, and the degree of diagnosis was extremely difficult for intraepithelial cancer patients. The clinical study was initiated and three cases of intraepithelial carcinoma were identified. The Japanese Society of Digestive Diseases, the Japanese Society of Digestive Endoscopy, and the Japanese Society of Clinical Medicine have all been invited to participate in the study. This clinical study is scheduled to be conducted after 4 years and in the interim.
