项目摘要

今年度は、昨年度に実施した文献連ビューについて「保健活動における地域診断の実践に関する文献検討-過去5年間の和文論文から-」と題して、日本地域看護学会第25回学術集会にて発表を行った。地域診断の実施者は、保健師のほか、学生、研究者、医療福祉職や住民といった多様な職種により行われ、報告されていた。また、研究方法としては実態把握や文献検討が多くを占めていた。実践上の困難は、「保健師」の実践に関する報告のみで、いかに現場の保健師が地域診断に困難を感じているかが明らかであった。困難の種類として、疫学統計手法に対する困難、日常の保健活動からの情報収集が困難といった「実践方法に関する困難」と、時間が取れない、助言が得られないといった「実施体制上の困難」などがあった。課題として、現場に即した地域診断モデルの開発や、職場の実施体制整備による実施率向上が挙げられていた。保健師が実践上の困難を感じている疫学統計的分析は、研究者による報告が多くあり、また、住民を含むその他の職種による地域診断の報告もあったことから、研究者をはじめ多職種とともに地域診断を実践することが課題解決に繋がる可能性が示唆された。本文献レビューの結果、多職種協働による地域診断において保健師が果たす役割を明確化するとともに、上記を踏まえた実践的地域診断モデル開発の必要性が再確認され、本研究課題で構築を目指す教育プログラムの方向性が示されたと考える。学会発表での参加者とのディスカッションでは、実践上の困難や課題の中に実践率向上の糸口があるだろうとの意見を頂き、今後のインタビュー調査ではこの部分を具体的に掘り下げながら進めていく方針とした。上記を受けて、インタビューガイドを修正し、調査実施に向けての準備をした。今後は、インタビュー調査を実施し、調査内容の分析を進める予定である。
