项目摘要

16歳から40歳までに発症する若年がんに対する治療や予防に関するエビデンスは少なく、ここ20年間の5年生存率の改善は認められておらず、若年がんに対する予防法の提言や治療標的の同定はアンメット・メディカル・ニーズが高い領域である。肺がんは一般的に喫煙などの環境要因が発症リスクに強く関与することが知られているものの、若年発症肺腺がんでは非喫煙者女性および進行がんが多いなど、他の世代とは異なる特徴が報告されている。したがって、喫煙などの環境曝露以外に遺伝的要因が関与している可能性が考えられる。これまで、アジアからの若年発症肺腺がんに特有の遺伝的要因に関する報告はなかった。当研究室では多施設共同研究により、若年肺腺がん345例由来の血液DNAを用いた全エクソンシークエンスおよび全ゲノムシークエンスを実施して、症例対照研究によりオッズ比で2倍以上の機能失活型に関わる生殖細胞系列バリアントを同定した。本研究では、既に候補として同定した機能失活型バリアントに着目して、①40歳以降の肺がん症例における機能失活型バリアントの頻度の算出（若年がん特異的であるか？）、②得られた頻度データからの生涯発症リスクの算出、③がんゲノムシグネチャーからの治療選択性の可能性を検討している。本研究により、5年生存率が改善されない若年がんに対する基盤的情報の構築を目指す。

When the laboratory conducted joint research, 345 cases of lung origin blood DNA were used in the study, and the ratio of functional inactivation was more than twice that of germ cell series. In this study, the frequency of functional inactivation was calculated from 40 years old to 40 years old. (2) calculation of the occurrence frequency of the disease;(3) discussion of the possibility of treatment selectivity of the disease. This study aims to improve the 5-year survival rate if the baseline information is available.
