本研究では,WHOが提唱した新しい齲蝕治療の概念である非侵襲的修復治療法(ART法)の小児歯科臨床における有用性について行動科学的および細菌学的観点から検討することを目的とした。平成12年度は,岡山大学歯学部附属病院小児歯科外来において,本研究の主旨に保護者の同意が得られた2〜6歳児32人(平均年齢4歳2か月)を対象とした。方法は,歯科不安に関するアンケートを実施した後,ART法に準じて充填処置を行い,光重合型グラスアイオノマーセメントを充填した。結果,細菌学的齲蝕活動性試験カリオスタットでのハイリスク者の割合は56%であった。診療台上での治療時間は平均12.3分であった。治療器具の使用に関して行動学的に何らかの問題が認められた者の割合は,刺激量が大きくなるほど増加していた。診療中の行動評価は,協力的な児が22%,診療に対し非協力的で阻害行動を示した児が44%であった。行動評価別に治療時間を比較すると,各群間に統計的に有意な差は認めなかった。平成13年度は,平成12年度に充填した対象歯の1年後の予後ならびに患児の歯科不安度と診療中の行動評価の変化を細菌学的ならびに行動科学的に検討した。結果,充填物脱落や辺縁破折等の予後不良症例は4例(12.5%)であり,他は充填物の変色などが認められたものもあったが臨床上特に問題なく経過していた。細菌学的にもカリオスタット検査の結果,ハイリスク者の割合は40%に減少していた。行動科学的検討では,患児の歯科不安度は平成12年度より軽減されており,行動評価でも治療に協力的に者の割合が増えていた。以上より,行動科学的に問題のある児,特に低年齢児に対して,ART法を適用することにより簡潔かつ効果的な齲蝕進行抑制治療が可能であることが示され,小児歯科臨床における有用性が示唆された。

The purpose of this study is to explore the new concept of caries treatment, the usefulness of non-invasive restorative therapy (ART) in pediatric dentistry, the role of mobile science, and the role of bacteriology.
