I目的 : 保健婦(士)が個々の要援助者に対して行うケース管理に関わる判断の構造を明らかにする。II方法 : 市町村及び保健所の熟練保健婦(士)が経験的に獲得し用いているケース管理に関わる判断の内容を記述し分析する。初年度の事例分析の結果、ケース管理の対象選定及び管理の継続・終結における判断に保健婦(士)のケース管理の特質が表れていることが確認されたため、この2側面の判断について、2人の熟練保健婦(40歳代,保健婦経験経験15年以上)の事例について分析した。平成13年度中に援助した事例のうち、「ケース管理対象者」と保健婦が判断した事例の判断根拠を、半構造化面接によりインタビュー調査した。逐語録から、前年度調査によって得られた判断要素項目(I)対象者・家族の状況変化と行動の予測、(II)地域資源の支援予測、(III)問題発生時・状況変化時の情報把握ルートの予測、に照合して検討した。III結果 : 分析の結果、(I)対象者・家族の状況変化と行動の予測として、(1)対象者の健康レベル、(2)対象者の日常生活自己管理能力、(3)対象者の他者支援受け入れ意志、(4)対象者の相談能力、(5)家族の健康レベル、(6)家族の日常生活自己管理能力、(7)家族の育児・介護能力、が、(8)家族の他者支援受け入れ意志、(9)家族の相談能力が、(II)地域資源の支援予測として、(10)地域資源利用状況が、(III)問題発生時・状況変化時の情報把握ルートの予測として、(11)対象者・家族と保健婦との信頼関係、(12)対象者・家族とサービス提供者・支援者との信頼関係、(13)サービス提供者・支援者と保健婦との信頼関係が、判断の項目として抽出できた。今後は、本研究によって抽出された判断項目の検証と、項目間の関連性の検討についてさらに研究を継続したい。

In the future, this study will focus on the identification of items and the correlation between items.
