一般に軟膏剤の多くは防腐剤が配合されており、細菌汚染を受けにくい。そのため国内外含めて、軟膏剤の管理方法・使用期限に関する規定・マニュアルは定められていない。しかし、軟膏剤どうしを混合すると、既製の製剤から組成が大きく変化するため、防腐剤の効果が十分とはいえず、細菌汚染する可能性がでてくる。また、軟膏剤を塗布する際に手指衛生が不十分な場合にも、細菌汚染する可能性が考えられる。実際に医療現場を院内感染対策チームでラウンドしていると、医療スタッフから軟膏剤の細菌汚染に関する問い合わせを受けるケースも少なくない。さらに本研究者は、病棟にて処置のために看護師が管理していた軟膏剤について、細菌汚染が認められた事例を経験している。こうした背景を踏まえた上で本研究者は、医療施設における軟膏剤の管理に関する規定・マニュアルを早急に整備する必要性があると考えた。そこで本研究では、まだ明らかになっていない、患者が自己管理する軟膏剤の細菌汚染の実態を調査することで、これまでの所見と合わせて、医療施設における軟膏剤の適正な管理方法や使用期限に関するエビデンスの構築を行った。23個の軟膏剤のうち、1個の軟膏よりStaphylococcus epidermidisが検出された。また細菌汚染が確認された軟膏剤については、塗布時には患者は手洗いのみ行い、直接指にとって塗布しており、患部を触った指で再度軟膏をとっていたことが聞き取り調査で明らかとなった。本調査の結果より、患者管理している軟膏剤において細菌汚染は少ないことが分かった。また、今回検出された菌は、皮膚常在菌であることから、患者が個人使用している軟膏剤においては、治療への軟膏剤細菌汚染の影響は少ないと考える。
