【目的】子宮体癌、子宮頸癌、卵巣癌の術後や再発症例に対する化学療法として繁用されるカルボプラチン+パクリタキセル療法は、日本癌治療学会編「制吐薬適正使用ガイドライン」ver. 1.2において中等度催吐性リスクに分類されるが、カルボプラチンを含むレジメンはオプションとしてアプレピタント(APR)の追加併用も推奨されている。APRは薬物代謝酵素CYP3A4の阻害薬であり、CYP3A4やCYP2C8で代謝されるパクリタキセルはAPR併用により血中濃度が上昇し、副作用の発生頻度が増加する可能性がある。効果と毒性の観点からその使用については検討を行い選択基準の作成の必要があり、本研究は必要なエビデンスの創出が目的である。【方法】カルボプラチン+パクリタキセル療法の制吐療法選択基準の作成を目的として、長崎大学病院産婦人科においてカルボプラチン+パクリタキセル療法(weekly TC療法)が施行された患者のデータを使用し、後方視的コホート研究を行った。【結果】・weekly TC療法における知覚神経障害発症のリスク因子解析知覚神経障害発症のリスク因子は、パクリタキセル治療歴なし(OR : 11.5, 95%CI : 1.6-238.1)、APRの併用あり(OR : 7.4, 95%CI : 2.0-33.5)であることを明らかにした。・weekly TC療法におけるAPRの副作用への影響の検討APRを併用した群(APR併用群)は、APRを併用しなかった群(APR非併用群)よりも倦怠感、知覚神経障害の発生率が有意に増加していた。【考察】本研究はWeeklyTC療法のみの検討しか行っておらず、今後はMonthly TC療法についても検討を行う必要がある。本研究よりAPRを併用した場合、知覚神経障害や倦怠感など患者のQOLに関わる副作用頻度を増加させることが示唆される。選択基準の作成には至っていないが、今後も継続して検討を行っていく。

[Results] Weekly TC therapy was used to analyze the severity factors of neurological impairment (OR : 11.5, 95%CI : 1.6-238.1) and APR (OR : 7.4, 95%CI : 2.0-33.5). Review of the effects of weekly TC therapy on APR side effects The incidence of burnout and cognitive impairment was intentionally increased in APR combination groups (APR combination group) and APR combination groups (APR non-combination group). [Investigation] This study is necessary for the study of weekly TC therapy and Monthly TC therapy in the future. In this study, the frequency of side effects associated with cognitive impairment, burnout, and QOL increased when APR was used together.
