(目的)2009年よりアプレピタント(商品名 : イメンドカプセルカプセル【○!R】)は本邦でも承認となった。また、従来の制吐剤と比較し半減期が長く、遅発性の悪心・嘔吐に対しても効果が期待できるパロノセトロン塩酸塩(商品名 : アロキシ注【○!R】)が開発され2010に国内で認可された。造血幹細胞移植、骨髄移植では前処置として大量化学療法が用いられる。これにより生じる悪心・嘔吐の発現率は極めて高い。一方で、大量化学療法施行の際の制吐療法についてはデータが不足しており最適な治療が行われているとは言い難い。そこで本研究では大量化学療法時の悪心・嘔吐を抑えQOLを向上するための最適な制吐療法の組み合わせを見出すことを目的とし、大量化学療法時のアプレピタント、パロノセトロンの制吐効果への寄与を確認することとする。(方法)2012年4月1日から2013年7月20日までに自家末梢血幹細胞移植(AUTO-PBSCT)にて大量メルファラ療法を行った患者を対象としレトロスペクティブに調査を行った。主評価項目は全観察期間における嘔吐なしの患者の割合とした。副次的評価項目として化学療法投与中および投与後における嘔吐なしの患者割合、化学療法施行中および施行後におけるcomplete response(嘔吐なし、救済治療なし)の患者割合を評価した。救済治療は悪心があった場合の追加の薬剤投与と定義した。(成果)2012年4月1日から2013年7月20日までにAUTO-PBSCTにて大量メルファラン療法を施行した多発性骨髄腫またはPOEMS症候群、原発性アミロイド―シスの患者は14名であった。主評価項目、副次評価項目双方で全観察期間の嘔吐の割合に差は認めなかった。しかし、全評価項目においてイメンドカプセル使用群で効果的な傾向がみられ、差を認めなかった原因としてはサンプルサイズの小さが原因となることが予測され、更なる症例の追加が必要と考えられた。

The secondary evaluation items include patient isolation during and after chemotherapy administration, patient isolation during and after chemotherapy administration, and patient isolation evaluation for complete response(vomiting, rescue). The definition of additional drugs for treatment of heart disease (Results) From April 1, 2012 to July 20, 2013, AUTO-PBSCT was administered to 14 patients with POEMS syndrome and idiopathic osteoporosis.
