1. 研究目的非小細胞肺癌に対して用いられるEGFRチロシンキナーゼ阻害薬であるタルセバ錠(一般名 : エルロチニブ塩酸塩錠)は、PPIによる胃内pHの上昇により胃内での溶解度の低下を引き起こしAUCが約46%低下することが知られている。よってタルセバ錠とPPIを併用すると、タルセバ錠の血中濃度が有効に保たれない可能性が考えられる。そこで本研究はリンゴジュースを用いたタルセバ錠の簡易懸濁法により、PPI併用によるタルセバ錠の吸収低下を回避できるかを検討した。2. 研究方法1. エルロチニプ測定方法の確立2. 簡易懸濁したタルセバ錠の懸濁性および胃環境での溶解性の比較A. 弱酸性およびアルカリ性溶液で簡易懸濁したタルセバ錠を作製する。B. 胃内に近い条件(pH1～1.5)での簡易懸濁タルセバ錠の溶出性の検討3. 研究成果1. HPLCを用い、エルロチニブ測定方法を確立した。2. タルセバ錠は酸性条件で溶出性が増加することから、リンゴジュースで懸濁したところ、水道水と比較し4℃および25℃条件下ともに溶出率の有意な増加が認められた。また、リンゴジュースで懸濁した懸濁液を第1液に溶解させたところ、水道水に懸濁させたものと比較し有意に溶解度が上昇した。今研究はPPI服用中におけるタルセバ錠の吸収抑制に対し、リンゴジュース等の酸性飲料水に溶解することで吸収抑制が回避できることを示唆した。

1. Objective: To investigate the effect of EGFR on non-small cell lung cancer (NSCLC). The increase of gastric pH and decrease of gastric solubility due to PPI were observed. AUC was about 46%. The possibility of maintaining the blood concentration of the drug in combination with PPI is discussed.
