项目摘要

近年, 新たな抗がん剤の開発に加えて支持療法が進歩したことによって, がん患者には外来通院治療が可能となった。その結果として患者のQOLは向上したが, 副作用は完全に消失したわけではない。口腔乾燥や粘膜障害, 日和見感染などによる口腔機能への悪影響は依然生じている。本研究では, 外来がん治療患者の初回利用者において, 口腔に発症する痛みが外来で生活を始めたがん患者のQOLにどのような影響を与えているかを調査して実態を知ることを目的とした。対象は, 2016年8月～10月末までに岡山大学病院腫瘍センターを受診し, 研究への同意が得られた患者98名である。方法は, 1. 口腔に関する問診(14項目), 2. 口腔内の疼痛の評価(Numerical Rating Scale), 3. 簡易検出培地による口腔カンジダ菌検査, 4. 口腔・咽頭疾患に関する疾患特異的尺度GOHAI(日本版)を用いたQOL評価, である。(倫理審査承認#1606-040)調査の結果, 対象患者の39%は65歳以上の高齢者であった。興味深いことに, 口腔カンジダ菌の感染量が多いとQOLが低下する傾向があった。しかし, 口腔カンジダ菌の感染量と疼痛との相関はなかった。QOL評価の各項目の詳細を調べると, 咀嚼, のみこみ, 思い通りにしゃべる, 楽に食べる, しみる感, などの5項目は, 口腔カンジダ菌の感染量と相関があった。がん治療中の患者にとって, 口腔の諸機能の低下は治療や日常生活の意欲低下につながりかねない。日和見感染である口腔カンジダ症は, 歯科受診で発見が容易である。がん治療患者も高齢化が進んでいるので, 積極的な歯科的介入や口腔機能低下を早期に発見して対処することは患者のQOLの向上に必須である。

In recent years, the development of new anti-cancer drugs and support therapy has advanced, so that patients can be treated in hospitals.
