研究目的造血細胞移植期に頻発する口腔粘膜障害は,耐え難い疼痛を引き起こすとともに感染のリスクを高める。私は,移植期の口腔内管理を現場の医療スタッフと行うとともに,臨床研究を行なってきた。がん対策基本法の施行により,「がん看護実践に強い看護師」の育成研修が実施され,苦痛に対する適切なアセスメントと,症状コントロールが目的の一つとなっている。患者や現場でケアを支援している看護師が,より安全で効果的なケア方法を理解し実践できるように,具体的なアセスメントの方法や口腔内管理方法の動画映像を編集し,実践に即した口腔内の管理の教材を作成する。動画等を用いた教材を利用し,充実した口腔ケアを展開する。移植患者の粘膜障害の重症度やモルヒネ使用量に及ぼす影響について調査する。研究方法本院で過去6年間に造血移植前処置を受けた250名を対象とした。移植の種類は,血縁者間骨髄移植:20名,末梢血幹細胞移植:96名,非血縁者間骨髄移植:73名,臍帯血移植:61名であった。移植前処置開始から生着まで連日,当血液内科作成の口腔内観察表でのモニターを継続し,CTCAE v3.0を基準に粘膜障害の程度を評価した。各患者の移植期の最大スケールを求め,gradeの経緯とモルヒネ使用の変化を調べた。また,スタッフの口腔内をモデルとし,具体的なアセスメントや口腔内管理方法の動画映像を編集した。結果及び考察粘膜障害は,過去6年間で,重症の割合が減少する一方で,軽症の割合が増す傾向にあった。モルヒネ使用は重症化した時点での使用量が増す傾向にあったが,口腔内痛に限局できるものは少なかった。具体的なアセスメント方法や口腔内管理方法を動画映像に収めることにより,スタッフ勉強会での評価は高く,関連病院との合同研修会にも利用された。今後は,関連病院とも連携し,移植期の患者に対し,実践に即したより効果的で安全な移植期の口腔感染管理対策への構築につながるかもしれない。

Objective: To study the frequency of oral mucosa damage during hematopoietic cell transplantation. Private, transplantation-stage intraoral management, on-site medical care, clinical research. In response to the implementation of the Basic Law of the Policy, the training of "strong nurses in nursing practice" has been carried out, and the pain has been properly addressed, and the symptoms have been addressed to the purpose. To compile animation images of specific methods and methods of intraoral management, and to create teaching materials for intraoral management in practice. Animation and other teaching materials are fully utilized. The severity of mucosal damage in transplant patients was investigated in terms of the amount of tissue used and the impact of the disease. Methods 250 patients were treated before hematopoietic transplantation in our hospital during the past 6 years. The types of transplantation were as follows: bone marrow transplantation between patients with hematologic disorder:20, peripheral blood stem cell transplantation:96, bone marrow transplantation between patients without hematologic disorder:73, umbilical cord blood transplantation:61. The degree of mucosal damage was evaluated on the basis of CTCAE v3.0 when the intraoral examination table prepared by the Department of Hematology was established several days after the start of pre-transplant treatment. The maximum duration of transplantation for each patient was determined, and the grade of transplantation was adjusted. The animation image of the specific oral cavity management method is compiled. The results showed that the severity of mucosal lesions decreased and the severity of mucosal lesions increased in the past 6 years. The amount of oral pain tends to increase at the time of severe use, and the amount of oral pain is limited. The specific methods of oral administration include animation mapping, evaluation, and contract training. In the future, related hospitals will continue to work closely with patients during the transplant period, and the implementation of effective and safe oral infection management strategies during the transplant period will be carried out.
