项目摘要

近年,大病院では栄養管理を含む電子カルテシステムが導入されているが,栄養評価よりも情報管理を目的に開発されている。さらに,導入・保守が非常に高価である。本研究の目的は多くの中小療養医療機関で安価に導入できる栄養評価に重点を置いた栄養管理システムの開発である。本研究で開発したシステムは(1)栄養評価・必要量算出,(2)処方設計,(3)患者情報管理の3部門より構成される。(1)で患者の年齢,身長,体重,上腕三頭筋皮下脂肪厚,上腕筋周囲長,膝高よりHarris-Benedictの式等の3つの方法を用いて基礎代謝量が瞬時に算出される。そして,活動係数,傷害係数を任意に変更することより一日必要栄養量が容易に算出可能である。さらに,アミノ酸(蛋白)量,脂肪量,糖質量,水分量,塩分量をNPC/N比や脂肪割合などを変更することにより瞬時に算出可能である。(1)で算出された一日必要栄養量は(2)で静脈栄養管理の場合は注射剤の処方設計,経腸栄養管理を行う場合は日本人の食事摂取基準(2005年度版)の基準値と比較しながら適切な経腸栄養剤の選択,さらに,静脈栄養,経腸栄養,食事が混在する場合は各々の栄養療法で投与・摂取される栄養量を一括で算出可能である。また,褥瘡患者において経腸栄養剤を使用する場合,褥瘡治癒で不足している栄養素量を算出可能とした。(2)を使用するために輸液および経腸栄養剤のデータベースを独自に構築した。構築したシステムを2症例に使用した。症例1は経腸栄養剤の選択,症例2は入院時から退院時までの摂取栄養量の算出に使用し,一定の有用性を確認でき,論文として発表した。さらに,薬学生の病院実習において,医療現場で栄養輸液の適正使用が行われているかどうかを教育するために,栄養評価から必要栄養量の算出,さらに輸液設計までを本システムを活用して栄養療法の実際を体験学習させ,理解を深めることが可能であった。

In addition, when intravenous nutrition, enteral nutrition, and food are mixed, it is possible to calculate the nutritional amount of each nutritional therapy. When a bedsore patient uses a nutrient, the amount of nutrients needed to heal the bedsore may be calculated.
