【目的】本研究では、視点計測機を用いて薬剤師の散薬調剤時における視点の動きから処方箋等の記載項目や散薬調剤時の薬瓶名や薬剤秤量値の認識度等を数値化して解析することにより散薬調剤における調剤事故防止に活用することを目的に実施した。【方法】視点計測機には、瞳孔/角膜反射追跡装置を装着した帽子に動画記録装置を接続した機器を用いた。具体的には、本研究の被験者として同意の得られた薬剤師10名(経験年数9.4±8.2年)に機器を装着して、散薬調剤中の視点の動きを動画中に記録し、処方箋や薬瓶名及び秤量中における秤量値等の確認状況を解析した。計測では、過去のインシデント事例を参考に実際の処方内容と同じ模擬処方箋を3枚用いた。それぞれの処方内容は薬剤及び総秤量重量(6g)を同じとしたが、用法、用量及び投与日数を変えた。調剤は散薬監査システムの使用の有無に加え、使用時は処方オーダー連携の有無を比較するため計3通りで行った。【成果及び考察】処方内容及び薬袋の記載内容は全ての秤量開始前に確認されていた。薬瓶名は必ず秤量前後に確認されており、総確認回数は処方オーダー連携型の監査システム使用時が平均3.5回だったのに対して、連携無し(使用せずを含む)では4.4回であった。確認回数の増加は手にした薬瓶が処方薬剤と機械的に照合できないため、より注意が払われたものと考えられる。被験者の内2名は秤量最中でも天秤上で手にした薬瓶名を確認していることも判った。一方、秤量値確定後の確認回数は平均2.8回であり、処方箋内容の差は無かった。また、計測後に処方内容を聞き取り調査したところ正答率は56%であり、処方内容確認後は確認の比重が薬剤名と総重量に移行していると考えられた。調剤中の視点の動きを観察することにより、薬瓶名と秤量値の確認が複数回行われていることを客観的に示すことができた。また、処方内容と薬瓶名、総秤量値を機械的に照合する処方オーダー連携型監査システムが無い場合は、薬剤師がより慎重に作業を進めていると考えられ、同システムは、調剤ミス防止だけでなく薬剤師の心理的負担軽減にも有用と考えられる。
