【目的】注射剤を末梢血管から投与する際、しばしば静脈炎等の血管障害を引き起こすが、静脈炎の発現により治療の変更や中止を余儀なくされると共に、重症化した場合には血管周囲組織の炎症や壊死にもつながることから、実臨床における重要な課題の一つとされている。一方で、多くの注射剤では静脈炎回避のための取り組みが十分になされておらず、静脈炎が発現した際にステロイド等による対症療法が行われているのが現状である。本研究では、注射剤により引き起こされる静脈炎等の血管障害について、患者背景や投与条件等の詳細な解析を行い、注射剤による静脈炎発現の危険因子を明らかにすると共に、発現要因に基づく静脈炎回避策の立案および確立を目指した。【方法】本研究は九州大学医系地区部局臨床研究倫理審査委員会の承認を得た上で実施した。2009年1月から2011年12月までの間に九州大学病院救命救急センターICU病棟においてニカルジピン注射液を投与された患者の中で、対象となった41名について、血管障害の詳細を電子カルテより抽出し解析を行った。【成果】ニカルジピン注射液投与による血管障害発現に関与する因子について多重ロジスティック回帰分析により解析を行った結果、総投与量および、投与時間、投与速度において、障害発現群と非発現群との間に有意差が認められた。特に、投与時間が24時間以上かつ投与速度が45mL/h以上の場合に血管障害の発現頻度が高く、受信者動作特性(ROC)解析の結果、カットオフ値は投与速度が20.43時間、投与速度が44.72rnL/hであった。すなわち、ニカルジピン注射液による血管障害を回避するためには、添付文書の推奨濃度を遵守したうえで、投与時間を20時間以内かつ投与速度を45mL/h以内に設定すべきであることが明らかになった。

[Methods] This study was approved by the Clinical Research Ethics Committee of the Regional Department of Medicine of Kyushu University. From January 2009 to December 2011, Kyushu University Hospital Life Saving Center ICU Department administered injection to 41 patients and analyzed vascular disorders in detail. [Results] The factors related to vascular damage occurrence caused by injection were analyzed by multiple regression analysis. The results showed that the total dosage, time, speed of injection, and the intentional difference between the occurrence and non-occurrence of vascular damage were identified. Especially, the frequency of occurrence of vascular damage is high when the throwing time is more than 24 hours and the throwing speed is more than 45mL/h. The result of receiver behavior characteristic (ROC) analysis is high. The throwing time is 20.43 hours and the throwing speed is 44.72 rnL/h. The injection time is within 20 hours, the injection speed is within 45 mL/h, and the injection time is within 45mL/h.
