项目摘要

【研究目的】本研究では, 小児における口腔機能の発達と全身の協調運動の関連を検討し, 口腔機能障害治療の新しいアプローチを見出すことを目的とする.【研究方法】4-8歳の小児の協力を得て, 口腔機能の一つとして重要である構音の評価, および全身の協調運動の評価を行う. 構音については「構音検査法」を行い, 対象児の構音の特徴を把握する. 協調運動については保護者にThe Developmental Coordination Disorder Questionnaire (DCDQ)の記入を依頼する. 主成分分析を用いて, 構音の特徴とDCDQのスコアが相互にどのように関連するのかを比較検討する.【研究成果】73名の対象児に面接し調査を行った. 41名の対象児において正常から逸脱した構音(成長過程のものも含む)が認められた. DCDQのスコアとの比較(主成分分析)では協調運動の総合力と構音の習得に関連があることが示唆された. また, 全身運動と微細運動の得意分野・不得意分野のバランスが, 構音習得に影響を及ぼす可能性も考えられた.構音は口腔機能の一部であるが, その習得について全身の協調運動との関連を検討した研究はこれまでになかったが, 本研究の結果は, 両者には何らかの関係が存在する可能性を示している. 本研究の成果はこれからの構音障害に対する治療的アプローチの新展開の発端となるものと思われ, 口腔機能・構音習得と協調運動の関係をさらに詳細に検討することの意義が示された.なお, 研究成果については平成26年3月にアメリカ口蓋裂学会においてポスター発表を行った.

[Objective] This study aims to explore the relationship between the development of oral function in small children and coordinated movement of the whole body, and to explore new ways to treat oral dysfunction. [Methods] 4-8 years old children's coordination, oral function, articulation, and body coordination were evaluated. To grasp the characteristics of phonology of the image, the method of phonology detection is carried out. The Developmental Coordination Disorder Questionnaire (DCDQ) is recorded in the database. Principal Component Analysis (PCA) is used to analyze the correlation between the structural features and the DCDQ. [Results] 73 pairs of face to face investigation. 41 names and images in the middle of the normal transition from escape to structure (growth process and the end of the body) to recognize. Comparison of DCDQ's differences (principal component analysis) between coordinated motion and phonological acquisition. The effects of whole body movement and fine movement on articulation acquisition and the possibility of articulation acquisition are examined. The results of this study indicate the possibility of the existence of a relationship between phonology and oral function. The results of this research provide new insights into the treatment of articulation impairment, and demonstrate the significance of a detailed discussion of the relationship between oral function, articulation acquisition, and coordinated movement. In March 2006, the Institute for Research and Development launched a research project.
