【背景・目的】ファーマシューティカルケアを担う薬剤師による吸入指導の重要性はますます高まっている。効果的・効率的な指導のためには, 指導方法の標準化, 関わる医療者の情報共有・伝達システムが必要であるが、そのためのツールやシステムが十分に整備されていない状況が続いていた。我々は2010年に群馬吸入療法研究会を立ち上げ連携プロセスを確立し, 患者が確実に質の高い吸入指導が受けられる環境を整えた。しかし, 吸入連携開始後, 吸入指導の継続が困難なケースが生じていた。問題点の一つとして, 薬剤師による患者アウトカム(QOL)の評価が十分でなく, 喘息や慢性閉塞性肺疾患のQOL評価資材も疾患により様々で統一した評価が困難なことが挙げられた。服薬アドヒアランスの低下は長期薬物療法が必要な慢性疾患患者にとても多い。2013年9月に慢性疾患患者における統一した服薬アドヒアランスASK問診表(Adherence Starts with Knowledge)の日本語版が発表された。ASKは既存のアドヒアランス・スケールと異なり, 症状コントロールだけでなく治療に対する姿勢や医療者とのコミュニケーションや治療満足度を評価できる。日本において薬剤師がASKを用いて患者のアドヒアランスを評価し治療に反映させる試みはなされていない。本研究では、吸入指導において服薬アドヒアランスASK問診表を用いてQOLを評価し治療に反映させることで、ASKスコア、指導回数がどのように変化するかを調査した。【研究方法】慢性呼吸器疾患患者に対し吸入連携パスを作成し薬剤師がASKを用いてアドヒアランス評価を行い指導前後のASKのスコア変化を評価する。【成果】現在ASKを組み込んだ吸入連携パスを作成中である。今後も研究を継続しASKスコアの変化、既存のQOL評価資材を用いた場合に比べ指導回数、呼吸機能がどのように変化するかを調査する。
