肺移植後の薬剤管理においては、カルシニューリン阻害薬による拒絶反応抑制とイトラコナゾールによる肺アスペルギルス症感染予防の両立が重要となる。両剤併用時には肝薬物代謝酵素CYP3A4を介した薬物相互作用による副作用リスクの増大を回避するため、カルシニューリン阻害薬の適切な減量が必要となるが、薬物相互作用に個体差が大きく、明確な用量設定根拠は未だ得られていない。そこで、肺移植患者におけるイトラコナゾール併用時のカルシニューリン阻害薬減量ノモグラムの作成を目的に臨床データの解析を行った。京都大学医学部附属病院において2008年以降に肺移植を施行した患者のうち、イトラコナゾールおよびカルシニューリン阻害薬の血中濃度を同時に測定した62名を対象に、タクロリムス及びシクロスポリンの血中濃度/投与量比に対して、イトラコナゾールの有無、投与量、血中濃度が与える影響をNONMEM解析にて検討した。イトラコナゾールと代謝物であるOH体の血中濃度は良好な相関を示したことから、代謝速度の個体差は小さく、イトラコナゾール血中濃度のばらつきは吸収に起因している可能性が考えられた。タクロリムスの血中濃度/投与量比はイトラコナゾールの併用により平均4.2倍に上昇したが、イトラコナゾールの投与量や血中濃度との相関性は低かった。一方、シクロスポリンの血中濃度/投与量比はイトラコナゾールの併用により平均3.3倍に上昇し、イトラコナゾールの投与量とは相関しないものの、イトラコナゾールの血中濃度と相関を示した。従って、肺移植患者においてイトラコナゾール併用時に、タクロリムスの場合は投与量を約25%に、シクロスポリンの場合は約30%に減量することが望ましいと示唆された。さらにシクロスポリンの場合には、イトラコナゾールの血中濃度がカルシニューリン阻害薬減量ノモグラムを作成する上で重要な要素になる可能性が示唆された。
