歯周病の予防および予後を考える上で,歯周病原因菌の定着および伝播を把握することは重要である。今回我々は,歯周病原因菌の定着する年齢層と家族内伝播を明らかにすることを目的に実験を行った。対象は本学小児歯科診療室を受診した男児12名(平均年齢10歳8ヶ月)女児(平均年齢9歳4ヶ月)および2家族(2家族とも父,母,女児)である。Gingival indexが1以上の歯肉炎を認める歯肉縁下(平均ポケット深さ2.3mm)からプラークを採取し,嫌気条件下で培養を行い,細菌の分離,同定を行った。更に,染色体DNAを抽出し,制限酵素で,digestionした後,電気泳動を行い,泳動パターンより菌株の同一性を決定した。結果は,以下の通りである。プラークを採取した小児17名のうち歯周疾患に関連しているといわれている黒色色素産生性Bacteroides(BPB)が検出されたのは,男児12名中3名であり,女児は5名中5名全員であった。それらのBPBの種を同定してみると,全て,思春期性歯肉炎の原因とされているPrevotella intermediaであった。また,DNAフィンガープリントにより,菌株の同一性を調べたところ,全員1種類の菌株しか保有してなかった。成人における歯周疾患の原因菌として最重要菌といわれるPorphyromonas gingivalis,前歯春期性歯周炎に深く関与するといわれるActinobacillus acitinomycetemcomitansは,今回は検出されなかった。また家族内伝播を調べるために,2家族からプラークを採取したが,いずれの家族も母親と女児からは,BPBが検出されたが,父親からは検出されなかった。

It is important to understand the causes of dental diseases. In this paper, the causes of disease in the family are determined. 12 male children (average age: 10 years 8 months) and 2 female children (average age: 9 years 4 months) were examined in our pediatric clinic. Gingival index above 1 for dental inflammation (average depth of 2.3mm), culture and isolation of bacteria under ambient conditions. Furthermore, chromosome DNA extraction, restriction enzymes,digestion, and electrical mobility determine the identity of the strain. As a result, the following is the case. 17 children were found to be associated with peripheral diseases, 3 out of 12 boys and 5 out of 5 girls were found to be associated with pigment producing Bacteroides(BPB). The cause of sexual arousal is Prevotella intermedia. The identity of the strain was adjusted, and all strains of 1 species were retained. Porphyromonas gingivalis, Actinobacillus actinomycetemcomitans and Actinobacillus actinomycetemitans are the most important causes of adult periodontitis. The family is divided into two families: the mother and the daughter, the BPB and the father.
