1市の高齢者支援における保健婦活動を調査対象とし、活動の中で、看護機能がどのように発揮されているか抽出することを試みた。看護機能について明らかにすることを目的とするので、活動からは保健婦の行動のみでなく、その活動の意図を忠実に取り出した。また、それらの活動による対象高齢者や市民への影響を合わせて調査し、看護の機能が地域の高齢者支援にどのように貢献するのか考察した。今年度は1市の老人保健法に基づく機能訓練事業における保健婦活動について調査を行った。対象市の機能訓練事業では、中途障害者や虚弱高齢者の社会生活拡大や閉じこもり防止等を目標としている。特にコミュニティーを基盤として公民館等を会場にしたリハビリ教室を全市で十カ所開いている。これらの活動における保健婦の意図の根幹は、「障害者や高齢者に自然に支えあえるような地域づくり」であった。一地区のリハビリ教室発足の準備期から活動が軌道に乗るまでの期間の保健婦の具体的な活動内容と働きかけの対象を調査したが、全ての行動は前述の意図に基づくものであった。その期間中の住民の行動や言動をみると、「教室以外に町で会ったとき自然に声かけでき交流が始まった」「交番の警官や自転車屋さんが交通整理や車椅子の修理をしてくれるようになった」「ボランティアが障害者について特別視しなくなった」などがあった。これらの結果から、地域の高齢者支援において、高齢者自身への直接的ケアのみならず、これらを取り巻く地域の人的物的環境に働きかけ、弱者としての高齢者が住みやすい町づくりに看護機能が貢献していることを確認した。地域を基盤にした機能訓練事業の展開は、平成8年度よりB型機能訓練事業として老保法にも位置づけられているが、対象市では昭和62年度から,前述の目的で地域基盤の展開がなされており、施策につながるような市民ニーズを先駆的に発掘し事業展開させていく機能も看護機能として注目される。
