本研究の目的は、地域在住高齢者に対して、機能的バランス能力を測定し、歩行能力や下肢筋力、転倒既往との関連性を分析し、その有用性を検討することである。歩行が自立している地域在住高齢者40名を対象に、過去6ヶ月間の転倒既往の有無、歩行速度、下肢筋力、重心動揺、機能的バランス能力を測定した。下肢筋力は、徒手筋力測定評価器を用いて、股関節屈曲、伸展、膝関節屈曲、伸展、足関節背屈、底屈の等尺性筋力を測定した。重心動揺は、重心動揺解析システムを用いて開眼・閉眼時の静的立位時重心動揺と前後、左右方向への動的重心動揺を測定した。機能的バランス能力については、Functional reach test(FR)、Timet up and go test(TUG)、Functional Balance Scale(FBS)を測定した。得られた値から、重心動揺と機能的バランス能力両者と、歩行速度、転倒既往、下肢筋力との関連性を分析した。統計学的分析の結果、以下の点が明らかとなった。1. 年齢を制御した歩行速度との偏相関係数では、重心動揺は有意な相関を認めなかったが、FR・TUG・FBSはいずれも有意な相関を認めた。2. 下肢筋力との関連性においては、重心動揺では有意な相関を認めなかったが、FBSとは有意な相関を認めた。3. 転倒既往の有無との関係では、閉眼立位時重心動揺は転倒既往者で有意な減少を認めたが、他の重心動揺では有意な差は認めなかった。機能的バランス能力では、FR・TUG・FBSはいずれも有意差を認めた。以上より、高齢者に対する機能的バランス能力評価は重心動揺測定に比較し、歩行速度、下肢筋力、転倒既往との関連性がより認められ、加えて測定が簡便なことから、その有用性が示唆された。今後、転倒発生の予測や転倒予防介入研究に活用できるものと考えられる。
