项目摘要

1.白癬合併喘息に関するpreliminary study研究代表者の担当する外来喘息患者の一部を対象に白癬合併の有無を検討した。その結果10例の足あるいは爪白癬合併例を見い出した。いずれも高用量吸入ステロイド薬を投与しても十分にコントロールできない全身ステロイド薬依存性の重症難治例であった。この10症例に白癬の治療目的で経口抗真菌薬itraconazole 150mg/日を投与したところ、全例で足・爪白癬の著明な改善が得られたが、うち4例では喘息症状やピークフロー値も明らがに改善し全身ステロイド薬を減量することができた。8週間の投与終了後も現在まで1年以上にわたって良好な状態を保っており、全身ステロイド薬は減量したまま経過している。特にこのうちの1例では、劇的ともいえる自覚症状、生活の質の改善を維持している。以上より、気管支喘息におけるTrichophyton感染の関与および抗真菌薬による治療の可能性が示唆された。2. 喘息患者における白癬合併頻度、Trichophytonに対する感作の頻度の検討喘息患者における白癬感染の合併や、Trichophytonへの感作(特異IgE抗体価の測定と抗原皮内反応により評価)の頻度を明らかにする。感染や感作の有無と患者の重症度、ステロイド投与歴などとの関連を検討し、感染や感作を生ずる背景因子を明らかにする。白癬感染合併例では喘息、白癬の両者の発症時期と経過を詳細に問診し、両者の因果関係の解明を目指す。上記を目的として、研究代表者の担当する外来の全喘息患者を対象に研究を開始し、データを集積中である。

1. A preliminary study on the role of white fungus in wheezing was conducted on behalf of a group of patients with foreign wheezing. The results of 10 cases of foot and claw white fungus combined with cases of foot and claw white fungus were reported. In the middle of the day, high doses of inhaled drugs were used to treat severe cases of systemic drug dependence. In 10 cases, the treatment goal of white fungus was to improve the symptoms of white fungus in feet and feet by administering itraconazole 150mg/day. In 4 cases, the symptoms of wheezing were improved by reducing the dose of itraconazole. After 8 weeks of investment and termination, it is now more than 1 year old. The frequency of leukaemia co-infection in wheezing patients, the frequency of Trichophyton infection co-infection in wheezing patients, and the frequency of Trichophyton infection (determination of specific IgE antibodies, evaluation of antigen intradermal reaction) are also indicated.
