项目摘要

1.目的足白癬・爪白癬は頻度の高い表在性皮膚真菌症である。最近の抗真菌薬は高い治療効果を挙げているが,日本の環境や生活習慣から再発を繰り返す場合が多く,再発を防止し,治療効果を高めるようなフットケアが必要なのである。また,フットケアは日常生活の中で自己管理されることが必要であり,生活に取り入れるには簡便かつ心理的負担が少ないケァであることが重要である。そこで,5本趾型靴下着用が足白癬患者のフットケアとして有用であるか否かを明らかにすることを目的に,靴下の着用に関する調査を実施した。2.方法(1)対象:東京都区内の皮膚科外来に通院している足白癬患者の中で,本研究の趣旨や調査結果の取り扱い等の倫理的配慮について,書面及び口頭にて説明し,了承を得られた者12名を対象とした。(2)調査期間:2007年10月〜2008年3月(2006年10月〜2007年3月にも調査を実施)(3)調査方法:週6日間の診療日を,介入群と対照群にそれぞれ2:1に割り付けた。2群とも標準治療(テルビナフィン1日1回塗布)を受け,介入群は5本趾型靴下を,対照群には同一素材の普通型靴下を対象者1名にっき5足ずつ配付した。調査開始後は,1日8時間以上を目安に配付した靴下の着用を遵守してもちい,カレンダータイプのチェック表に着用の有無を記入するよう依頼した。評価は1ケ月後,3ケ月後に行い,皮膚科医による治療効果の判定と,研究者が作成した評価項目による総合評価とした。3.結果対象者は,介入群9名(男性3名,女性6名),対照群3名(男性1名,女性2名)であった。対象者の人数が少なく,今年度分単独での統計学的分析は困難であるが,今後前年度分と合わせて分析する。
