项目摘要

平成10年度は、「炭酸リチウム投与による上皮小体機能亢進症の誘発実験」を行った。4週齢JCL-SDラット(オス、、メス)を以下の群に分け、各々炭酸リチウムを投与した。A群:炭酸リチウム100mg/kgを胃ゾンデにて強制投与、B群:0.1%炭酸リチウム溶解液を飲料水として自由摂取、C群:0.05%炭酸リチウム溶解液を飲料水として自由摂取その結果、A群、B群は副作用のため長期投与例が設定できず、短期投与例のみの検討では、上皮小体機能亢進症の誘発は確認されなかった。一方、C群は、短期投与例、長期投与例の設定が可能であり、長期投与例で、オスラット、メスラットともに血清カルシウム値、Rat PTH値の有意な上昇を認めた。それとともに上皮小体の増大が確認され、上皮小体機能亢進症が誘発されたものと考られた。また短期投与例、長期投与例ともに明らかな副作用は確認されなかった。腎組織の検討では、メスラットにおいて、皮質髄質境界付近の尿細管内に、HE染色で青紫色に染色されるplaqueを認めた。plaque内物質は、各種染色態度から、リン酸塩の存在が推定され、上皮小体機能亢進症によって生じた結石原基ではないかと考えられた。本実験系は、比較的、生理的条件下で上皮小体機能亢進症を誘発することが可能な点で、従来のモデルどは異なっている。本実験系で生じた上皮小体機能亢進症を詳細に検討することで、上皮小体機能亢進症の進展過程やそれによって生じる尿路結石の生成過程-の解明に寄与するものと思われる。本研究結果は、第86回日本泌尿器科学会総会、第8回日本尿路結石症研究会で発表し、日本尿路結石症研究会においては、日本尿路結石症研究会奨励賞を受賞した。
