先天性難聴児への人工内耳の適応基準の一つに「補聴器による聴能訓練が無効である」ことが掲げられている。しかし、抽象的な表現であり、具体的な検査法と検査所見は併記されず、実際に人工内耳を適応する場合には各聴能担当者の判断によるところが大きい。このようになった背景として、幼少児を対象とした検査として、聴取能力を反映する検査は多種にわたりあるものの、人工内耳の適応を決める上で重要となる内言語を評価する検査法に適切なものがないことは大きく関与している。上記のような現状を踏まえ、人工内耳の適応を決定するための言語評価法を開発した。とくに、聴性行動をはじめとする表出面に分析の重点をおき、他の聴取検査の結果と組み合わせることによって、内言語能力を評価することを目的にしている。検査対象は5歳児までを想定し、検査はあらかじめ作成したビデオ教材を見せたときの対象児の表出行動をビデオ記録し、これを分析するものである。現在、2歳の先天性難聴児、1歳の後天性難聴児(いずれも人工内耳埋込予定あり)を対象に定期的に検査を実施している。また、コントロールとして、4歳、2歳、0歳の健聴児3名も同様の検査を実施している。最終的な獲得言語能力を評価できる年令を6歳とすると、1歳の難聴児について結論がでるのは5年後であり、今後5年間は本研究を継続する予定である。なお、著作権の問題から検査内容を現時点で公表することはできない。現在、著作権保有者と交渉中であり、将来的には検査内容を公表する予定である。
