项目摘要

1 抗フォスファチジルセリン(PS)抗体、抗フォスファチジルエタノールアミン(PE)抗体、抗カルジオリピン(CL)抗体陽性患者の血清よりIgG分画を抽出した。PSを細胞膜外層にtranslocateさせた血小板を用いフローサイトメトリーによりその結合を検討した。活性化した血小板はアネキシンVの結合が可能であり(Generation of annexin V-positive platelets and shedding of microparticles with stimulus-dependent procoagulant activity during storage of platelets at 4C.Transfusion 40:420-427,2000.)、その血小板を用いて以上のうち陰性荷電リン脂ソに対する抗リン脂質抗体の結合を確認した。2 活性化血小板の研究は心不全患者を用いて検討も行った(Changes in platelet activation associated with left ventricular assist sysem placement.J Heart Lung Transplant,19:462-468,2000.)。さらにレーザー散乱光を用いた血小板凝集能検査により微少血小板凝集塊の産生が女性に多いことを見いだした。(Sex difference in platelet aggregation detected by new aggregometry using light scattering.Endocrin J,in press.)3 昨年度にスクリーニングした抗アネキシンV抗体のIgG抗体分画の性格を検討した。カットオフ値を非妊婦コントロールの4MoM以上に設定すると、反復流産患者の10.5%(25/238)および反復体外受精不成功患者の10.4%(5/48)に抗アネキシンV抗体は陽性であり、この頻度は非妊婦コントロールの5.0%(9/179)および妊婦コントロールの0.8%(1/120)に比べ有意に高かった。また、17名の抗アネキシンV抗体強陽性患者(7MoM以上)のうち抗PS抗体、抗PE抗体、抗CL抗体のいずれも認めないものは、10名(58.8%)であった。(Detection of anti-annexin V antibodies in recurrent pregnancy loss patients.J Autoimmunity,15:A64,2000.)
