Pulse wave velocity(以下PWV)は動脈硬化症の早期発見に有用であることが指摘されており、動脈硬化危険因子との関連が最近多く報告されている。しかし、いずれも断面調査による報告であり動脈硬化危険因子とPWVの関連を前向きに検討した成績はほとんど見られない。そこで教室で20年来継続している疫学調査である端野・壮瞥町研究から1991年、1992年に検診を受診した2,215名(男性949名、女性1,266名)のなかで2001年にも検診を受診した男性344名全員を対象に日本コーリン社のABIフォルムを用いて上肢と左右下肢間のPWVを測定した。91年92年(初診時)の平均年齢は58.8±9.1歳で初診時全員に早朝空腹時に身長、体重、座位随時血圧、空腹時血糖、総コレステロール、中性脂肪、HDLコレステロールを測定し、インフォームドコンセントを得て75gOGTTが施行された。降圧剤内服中の56名を含め132名(38.4%)が高血圧と診断された。境界型を含む耐糖能異常は糖尿病治療中の20名を含め58名(16.9%)であった。2001年の検診ではABIフォルムを利用し、ankle brachial pressure index (API)が0.9以下の下肢閉塞性動脈硬化症の者は13名(3.8%)で、上肢と左右下肢間で測定したPWVの平均値は正常血圧群1593.3cm/secに対し高血圧群で1892.3cm/secと有意に高く、また正常耐糖能群のPWVが1676.5cm/secに比して耐糖能異常群で1863.7cm/secと有意に高値であった。高脂血症においてはPWVに差異はなかった。PWVは初診時年齢の他、初診時の収縮期血圧、拡張期血圧および空腹時血糖とのあいだに相関がみられた。重回帰分析を行なっても初診時年齢、収縮期血圧、空腹時血糖はそれぞれ10年後に測定したPWVの独立した有意な説明変数となった。動脈硬化危険因子のフォローにPWVの測定を併用することは有用であろうと思われる。

Pulse wave velocity(PWV) is useful for the early detection of atherosclerosis and has been recently reported as a risk factor for atherosclerosis. The results of the cross-section survey were reported as follows: In the past 20 years, 2,215 patients (949 males and 1,266 females) were examined in 1991 and 1992. In 2001, 344 males were examined in 2001. The PWV between the left and right limbs of the Japanese Society was measured. In 1991 and 1992 (at the time of initial diagnosis), the average age was 58.8±9.1 years old. At the time of initial diagnosis, the length, weight, seat pressure, fasting blood glucose, total cholesterol, neutral fat, HDL cholesterol were measured in the morning, and the OGTT was performed in the morning. Among the 56 patients with hypertension, 132 (38.4%) were diagnosed as hypertension. 20 cases (16.9%) of diabetes mellitus with abnormal glucose tolerance were classified as boundary type, including 58 cases (16.9%). In 2001, there were 13 patients with lower extremity arteriosclerosis who had an ankle brachial pressure index (API) of less than 0.9 using ABI.(3.8%) The average PWV measured between upper limbs and left and right lower limbs was 1593.3cm/sec for normal blood pressure group and 1892.3cm/sec for high blood pressure group. The PWV of normal glucose tolerance group was 1676.5cm/sec for intentionally high glucose tolerance group, and 1863.7cm/sec for abnormally high glucose tolerance group. Hyperlipemia is a serious problem. PWV is associated with age at diagnosis, systolic blood pressure at diagnosis, diastolic blood pressure at diagnosis and fasting blood glucose at diagnosis. The blood pressure at the time of diagnosis, contraction and fasting were measured independently after 10 years. The measurement of atherosclerosis risk factors is useful.
