がん患者の苦悩を測定する40項目の自己回答式質問紙「がん患者苦悩尺度」の短縮版の開発を試みた。調査対象は176名で有効回答数は155(有効回答率88.1%)であった。分析対象とした155名のうち健康郡が90名、がん患者郡59名、精神科郡6名であった。調査期間は2000年9月〜12月の3ケ月間であった。がん患者郡の特性は男性6名、女性53名で年齢の範囲は32〜74歳、平均年齢は54歳であった。いずれも東京都内の大学病院の呼吸器・内分泌外来に通院する患者であり、疾患は乳がん32名、甲状腺がん25名その他が2名であった。調査内容は、すべての郡に本尺度・ZungのSDSを依頼し、がん患者郡のうち50名にはこれらに加えMAC Scale(Mental Adjustment Cancer Scale)への回答を得た。項目の洗練について、40項目における項目分析において相関の低い項目(.3以下)およびクロンバックのα値、因子分析(主成分分析・バリマックス回転)により寄与率が.3以下の項目、G-P分析により識別力の低い項目を吟味し、最終的に理論的構成概念から34項目に洗練した。「がん患者苦悩尺度短縮版」の信頼性について、test retestの結果、相関係数はr=.80(p<.001)で尺度の安定性が認められた(N=28)。34項目におけるクロンバックのαは90であり内部一貫性が認められた。基準関連妥当性について短縮版とSDSとの相関はr=.60(p<.01)であり、抑うつ尺度との基準関連妥当性が得られた。MAC Scaleとの相関はr=.11で無相関であった。この結果は、がん患者が体験する苦悩とがんへの適応は相関しない事を表している。因子分析により構成概念を吟味した結果4つの因子が抽出された。

The development of a shortened version of the 40-item self-answer questionnaire "Patient Suffering Scale" was tested. 176 respondents answered 155 questions (88.1% answered).
