平成15年度より本年度まで各年度の4月、10月に妊婦の喫煙状況に関する定点調査を施行し、1041名の結果を分析した。本年度は尿中コチニン量を用いて妊婦の喫煙状況を客観的に把握することを目的とした。1.妊婦の随時尿を質問紙と同時に909名より回収した。(回収率87.3%)2.尿中コチニン量の測定にはNicCheck I TEST STRIPSを用いた。3.受動喫煙の尿中コチニン量への影響を検討するために、自己申告で「喫煙の経験がない」と回答した妊婦で、調査前24時間の受動喫煙が10本以上の53名と1〜9本の33名の尿中コチニンを検討した。NicCheck値は全例で1以下を示し、受動喫煙の影響はNicCheck値1以下と判断された。また、NicCheck値2以上を示した症例は能動喫煙をしていると判断した。4.妊娠前(131名)もしくは妊娠後(170名)に禁煙したと回答した妊婦でNicCheck値が2以上を示した22名を虚偽の禁煙と判定した。虚偽の禁煙申告した妊婦は妊娠後禁煙した妊婦と比較して属性、妊娠背景、喫煙背景に差は認められなかった。5.虚偽の禁煙申告をした妊婦の割合は2003年2.5%、2004年5.9%、2006年11.6%と上昇していた。6.妊婦喫煙率は自己申告では2003年6.5%、2004年7.3%、2005年8.6%と若干の上昇にとどまったが、尿中コチニン量による補正を行った後の禁煙率は7.4%、9.1%、12.6%と大幅に上昇していた。7.2003年4月に健康増進法が施行され、妊婦やその周囲の人の禁煙が啓発されている。しかし妊婦の能動喫煙率の低下についての効果は認められず、むしろ妊婦が喫煙への罪悪感より虚偽の禁煙申告を行っている実態が明らかになった。今後、受動喫煙を含む喫煙プログラムの作成には、尿中コチニン量などの客観的な指標を用いる必要性がある。
