本研究の目的は、がん患者の疼痛コントロールに役立つ「痛み計」を開発することである。痛み計(23cm×6cm×2cm,160g)は、数値0から10が左から右へ約1cmの等間隔で並び、がん患者が痛みに合わせて、「全く痛くない」のレベルを示す0から「これ以上ないほどの強い痛み」のレベルを示す10の11段階の数値で、主観的な痛みの強さを表現するもので、その数値を記録・保管する小型で軽量な道具である。パソコンに接続すれば、痛みの24時間推移をグラフで一目瞭然に見ることができる。平成16年度当初は、触れるだけで数値が入力できる発汗計を組み込んだ皮膚電気抵抗型を作製したが、健康人と異なり患者の場合、指の皮膚面の乾燥により皮膚抵抗がほとんど無限大に近似して電極間電位が高くならないために数値が表示されない問題があった。次に、光学式に検知する光センサを用いて痛み計を試作し、特許を出願した。平成17年1月に倫理的に配慮してフィールド調査を始めたが、照明や日光の照度によって光センサの作動が不安定になる問題が新たに発生し、最終的に押しボタン式に変更した。対象は、大学病院の内科系病棟に入院中で、疼痛コントロールのためにオピオイドを使用しているがん患者10名で、平均年齢62.4(SD7.04)歳、疾患は肺がん9名、胃がん1名である。14日間の入力総回数は平均64.4(SD36.1)回で、スピアマンの相関係数からみて、痛み計の入力回数が多くなるにつれて、患者が出力されたグラフを見る頻度は多くなり、グラフを元に医師と話し合うことが増え、他の患者にも勧めたいという気持ちは高くなり、治療に参加している実感が高まった。これらの結果から、痛み計は、利用頻度が高い患者にとってセルフケアを促進するツールとなり得ることが示唆された。
