项目摘要

死別体験は、人間の限りある生命に直面した無力感や深い悲しみ、関係を築いた人を失い自らの身体の一部を奪われたような喪失体験をもたらす。患者の臨終に立ち会う病棟看護師は、その死別による深い悲嘆を体験するが、自らを専門職として律することで悲しみの感情を抑制し、自身の心の痛みにも気づく間もなく日々のケアに追われている。そのため、心の感受性が失われバーンアウトや離職に至る看護師も少なくない。近年、死別を含む‘否定的体験からの成長’が重視され、看護師においても死別体験を通して人として専門職として成長していくことが望まれる。本研究は、患者との死別による看護師の悲嘆を支援する具体的アプローチとして、デスカンファレンス(以下DC)に着目した。また、今まで個人に委ねられてきた死別体験から成長への対処行動を組織的に支援することを意図し、看護師が死別体験から成長に向かうDCの方略を明らかにすることを目的とした。初年度、予定通り、全国の緩和ケア病棟とがん診療連携拠点病院の消化器・呼吸器系病棟勤務の看護師長に対する調査結果から、「DCの効果的な運用手引き案」を作成することができた。一つに「DC開催の方略」として、DCの目的・時期・時間帯・参加職種・記録等について、現実的に開催しやすい方法を明示した。二つに「DC進行の方略」として、「発言しやすい雰囲気づくり」「安心して感情を表出できるための配慮」「無理のない肯定的な捉え方の共有」等の項目で構成し、分かり易く表記した。次年度、手引き案を十分に吟味し、一般病院勤務のがん看護専門看護師に質問紙調査を実施し、「運用手引き案」の妥当性を分析し検討する。最終年度は、実際の病棟に運用手引きの導入を試み、従来の場合と比較し、その効果を明らかにする。看護師のDC参加による効果も含め、最終的な「DCの効果的な運用手引き」を完成する計画であり、臨床への活用を期待できる。
