本研究は、生体肝移植レシピエントおよびドナーの体験からそれぞれにおける「移植」の意味を明らかにすることが目的であった。本年度は、これまでに明らかになったレシピエントとドナーの移植を決意するまでの体験をもとに、その関連性を検討した。また、レシピエントとドナーの移植手術後から1年間の体験についても明らかにする予定であり、それぞれ分析途中である。1.生体肝移植レシピエントとドナーが移植を決意するまでの体験の関連対象となったレシピエントとドナーは7ペアであった。レシピエントは40歳代から60歳代の男女で、病態は肝硬変、肝不全、肝細胞癌であった。ドナーは、20歳代から50歳代までの男女で、続柄は親子4組、配偶者3組であった。生体肝移植を受けるまでの過程は、まず、レシピエント、ドナーいずれにおいても「移植でしか助からないことの察知」のなかで、医師または家族による「移植の提案」がなされた。それによってレシピエントもドナーも「移植への葛藤」を生じ、「周囲の人々の支え」、「移植医療への信頼」によって「移植への決意」が行われ、その後も「他者との関係調整」が続いた。この移植の意思決定は、ドナー先行型、レシピエント先行型、共同意思決定型の3つのタイプに分けられた。特にレシピエントの病態が悪い場合は、ドナーを中心とした家族による意思決定が行われ、その後にレシピエントを説得する状況があった。2.生体肝移植レシピエントおよびドナーの手術後の体験研究開始時の対象者はレシピエント、ドナーそれぞれ5名であったが、体調不良や遠隔などの理由で術後1年後まで経過を追えたのはレシピエント2名、ドナー3名である。それぞれの移植後の急性期、回復期の体験を分析中である。
