项目摘要

近年長足の進歩をとげた遺伝子解析技術は様々なかたちで医療へ応用されており、それら(遺伝子診断・発症前診断・出生前診断・着床前診断など)をめぐって、社会的・倫理的問題が指摘されている。同時に、遺伝医療における心理的支援、とくに心理職等いわゆるメンタルヘルスプロフェッショナルによるそれの必要性および重要性が指摘されるようになっている。しかし、遺伝医療における心理的支援は、その実践においても理論化においても緒についたばかりであり、とくに臨床心理学の視座からの検討はほとんど行われていない。本研究は、当該領域の国内外の文献を検討しつつ、関係者へのヒアリングや遺伝医療現場での参与観察を行い、ワークショップを開催して当該問題全般の啓発と問題意識の共有を行うことで、臨床心理学的な今後の課題に対する一定の方向付けを目指すものである。全国の遺伝医療関連の診療部門では遺伝をめぐる医療相談が行われているが、心理的支援を必要とする来談者が少なくないことは、従来から関係者によって体験的に認識されてきた。医師や看護師にとどまらず、より専門的な立場からの心理的支援の必要性が指摘されるなか、従来の医療チームだけでは対応できない複雑な心理的問題をかかえた来談者が少なくないことを、遺伝相談外来で心理職として参与観察を行う中で再確認した。また、心理職が関与する遺伝相談外来は、2004年10月現在7施設であったが、2006年3月現在13施設に増えている。本年度の研究活動の一環として、これら施設の心理職に呼びかけワークショップを開催した。ワークショップでは、臨床心理学的観点からの事例の検討のほか、他職種との連携および役割分担のあり方をはじめ、医療チームの中で果たすべき役割について未だ整理な段階にあることが、再認識され今後の課題とされた。従来から準備してきた『遺伝医療と心理臨床』の刊行にもこぎつけることができ、次年度への足がかりを得た。
