项目摘要

研修医を対象として教育と教育成果の判定のために、臨床判断、医療安全、コンピテンシーを取り入れたコンピューターシミュレーションツールを作成した。18年度は、2次元の画像を基礎画面として、時間の経過とともに、痛み、バイタルサイン、診察、検査結果、画像、手技、病気の選択、および指示などが逐次、研修医の臨床判断能力を試す因子として変化するツールを作成し、その選択した記録は研修医評価のフィードバックに用いられるようにした。システム構築は(株)アーバン・コミュニケーションズは去年に引き続き依頼した。このデモシミュレーションツール画像の監視を行い、このツールを用いて研修医に実行させ、指導医の評価が可能であるかどうかを検証した。また、コンピテンシーと医療安全を含んだデモプログラムを外科学の臨床研修教育としてこのデモシミュレーション・ツールを用いて研修医に試行実施させた。6月27目に第一外科加藤俊二医師が、研修医のミニレクチャーで腹部救急のシュミレーションとしてが紹介した。臨床判断を取り入れたプロセス優位のコンピュータシミュレーションとして検査の手順と時間的観念をいれているので有効であった。研修医は診察から検査の手順を理解していないことがこのツールを使って判明した。知識不足とともに慣れていないためであることが明瞭になった。指導医からは、もう少し写真だけでも入れられるとすぐにも使えるという評価があった。今後の課題は操作性の向上、評価の精度の向上でさらなるシステム構築の改善が好ましい。米国サンディエゴで開かれた第6回医療シミュレーション学会でAssessment tool for resident using the clinical decision making simulation with patient safety成果を発表した(2006年1月)。
