项目摘要

本研究は、がん末期患者の緩和困難な苦痛症状に、温灸による看護介入を行い、苦痛症状軽減を図ることを目的としている。平成18年度は温灸療法の介入手順、質問用紙およびデータ収集用紙等を確定した。平成19年度は2施設(A大学病院とB緩和ケア専門病院)において看護介入実験を行った。平成20年度は上記2施設に加え、C一般病院でもデータを収集し、データ分析・まとめを行った。結果を以下に述べる。1. 対象者は50名(年齢63.10±12.31歳、男性・女性いずれも25名)で、A病院入院中患者が29名、B緩和ケア病院入院中患者が9名、C一般病院入院患者が12名であった。昨年度と同様の理由により、本年度も対象数を増やすことは難しかったが、介入方法は前年度の形式を継続した。2. 15分間の温灸療法の前後で、血圧、脈拍、呼吸数などに低下をもたらす場合もあった。これは温灸だけではなく、15分間の安静の効果も影響していると考える。主観的な症状である「気分」は3日目の温灸の前後および、連続した5日間の温灸の前後で有意に改善の傾向をもたらした。温灸の前後で「気分」の改善を認めたことから、特定の看護者が温灸という看護介入を通して一定期間関わることが、患者の心理側面や実存的側面に影響したとも考えられる。QOLについては、「体調」、「睡眠」において有意差が認められた。また全身状態(Performance Status; PS)の悪い患者に効果が認められる傾向があった。3. タキサン系抗がん剤の副作用によるしびれの改善が認められた事例を経験し、温灸による温熱刺激がしびれの症状緩和に効果がある可能性が示唆された。

This study aims at alleviating pain symptoms, nursing intervention, and pain symptom reduction in patients at the end of treatment. In 2018, the intervention procedure of warm moxibustion therapy, questioning paper and collection paper were determined. In 2019, two facilities (A University Hospital and B Relaxation Hospital) were established. In 2010, the Ministry of Health issued a report on the implementation of the plan. The results are described below. 1. There were 50 subjects (63.10±12.31 years old, 25 males and females), 29 admitted to Hospital A, 9 admitted to Hospital B, and 12 admitted to Hospital C. For the same reason as last year, the number of cases in this year has increased, and the intervention method has changed from the previous year. 2. Before and after 15 minutes of warm moxibustion therapy, blood pressure, pulse beat, respiratory rate and other low occasions The effect of the 15-minute silence on the temperature and humidity of the room was studied. There is a tendency to intentionally improve the symptoms of the main symptoms before and after warm moxibustion on the 3rd day, and before and after warm moxibustion on the 5th day. Before and after warm moxibustion, the improvement of "Qi points" was recognized, specific caregivers were involved in warm moxibustion, and nursing intervention was conducted for a certain period of time. The psychological bottom of patients and the bottom of existence were affected. QOL General Performance Status (PS) and Patient Status (PS) 3. The possibility of improving the side effects of anti-inflammatory drugs is also indicated.
