Study of nuclear DNA content and malignant potential in colorectal carcinoma.1) Microspectrophotometric DNA analysis was carried out in 76 cases of colorectal carcinoma. In result four types of DNA histogram patterns were obtained : In type I, the peak ranged from 1.6 to 2.4c (diploidy) and the DNA content over 4c was distributed by less than 20%. In type II , the peak was also diploidy and the DNA content over 4c was distributed by 20% or more. In type III , the peak appeared in the range from 2.5c upward (aneuploidy) and one over 4c was found by less than 20%. In type IV , the peak was aneuploidy and the one over 4c was found by 20% or more. Diploidy case was observed in 85.0% of the 13 cases in early colorectal cancer, while in type IV it was observed in only one case(7.7%). Lymphatic permeation was remarkable (ly_<2.3>) in 5 cases (11.6%) with type I and in 24 cases (55.8%) with type IV . Venous invasion was remarkable (v_<2.3>) in one case (6.7%) with type I and in 8 cases (53.3%) with type IV .2) Flow cytometric DNA analysis was carried out using paraffin-embedded materials of 143 cases of colorectal carcinoma. The ratio of DNA diploidy against DNA aneuploidy was one second. DNA ploidy pattern was not correlated with histological grade but with histological factors such as lymphatic permeation and venous invasion. Rate of lymphnode metastasis was higher in DNA aneuploidy (62.4%) than DNA diploidy (30.0%).At the primary operation, the rate of liver metastasis in DNA aneuploidy was 16.4% but in DNA diploidy was not observed. These results indicate that DNA ploidy pattern is related to lymphnode metastasis and liver metastasis. Furthermore, the 5-year survival rates in patients with absolute curative resection were lower significantly in DNA aneuploidy (62.5%) than DNA diploidy(92.9%).In conclusion microspectophotometric and flow cytometric DNA analyses are useful for evaluating the biological malignant potential and predicting the liver metastasis.
结直肠癌细胞核DNA含量与恶性潜能的研究1)对76例结直肠癌进行了显微分光光度法DNA分析。结果显示,DNA直方图有四种类型:Ⅰ型,峰值在1.6 ~ 2.4c之间(二倍体),大于4c的DNA含量分布在20%以下;Ⅱ型也是二倍体,DNA含量大于4c的占20%以上。在III型中,峰值出现在2.5c以上(非整倍体)的范围内,超过4c的非整倍体不到20%。在IV型中,非整倍体是最高峰,超过4c的占20%或更多。在13例早期大肠癌中,二倍体占85.0%,而在IV型大肠癌中仅1例(7.7%)。I<2.3>型5例(11.6%),IV型24例(55.8%),淋巴管浸润明显。I<2.3>型1例(6.7%),IV型8例(53.3%),静脉侵犯明显(v_)。2)对143例结直肠癌的石蜡包埋材料进行流式细胞术DNA分析。DNA二倍体与DNA非整倍体之比为1秒。DNA倍体模式与组织学分级无关,但与淋巴管渗透和静脉浸润等组织学因素相关。DNA异倍体组淋巴结转移率(62.4%)高于DNA二倍体组(30.0%),首次手术时肝转移率为16.4%而DNA二倍体组无肝转移。提示DNA倍体类型与淋巴结转移和肝转移有关。DNA异倍体组5年生存率(62.5%)明显低于DNA二倍体组(92.9%)。结论显微分光光度法和流式细胞术DNA分析对评估肿瘤的生物学恶性潜能和预测肝转移有重要意义。
