目的)骨髄異形成症候群(MDS)患者の中で骨髄中好酸球比率が5%以上の症例(MDS-EO)を7%見いだし、これらが特徴的臨床像および染色体異常を有することを報告した。よって本疾患における好酸球増多の機序を解明し、疾患群としての確率を試みた。結果)(1)MDS-EO患者8例の骨髄好酸球を透過電顕的に観察した。内3例では顆粒の形態異常および核と細胞質の成熟のアンバランスを認めた。(2)MDS-EO患者の内、芽球の増加を認めた3例よりcell sorterにて芽球を抽出し、各種サイトカイン存在下で短期培養した。内1例の芽球は、IL-3,IL-5,GM-CSF存在下で好酸球に分化した。これは光顕的および電顕的に確認した。(3)MDS患者とMDS-EO患者の血清サイトカイン濃度を比較検討した。IL-1β、IL-2,IL-3,IL-5,IL-6,G-CSF,GM-CSFには有意な差は認めなかった。(4)上記の芽球の増加を認めたMDS-EO患者3例の芽球の短期培養後の上清中のサイトカイン濃度には特に有意な上昇は認めなかった。(5)MDS-EO患者の内、特異な染色体異常を有する症例は膠原病様症状と間質性肺炎を呈することを見いだした。考察)MDS-EO患者は病因的には2群に分けられると思われる。第1群は、腫瘍性の好酸球増多である。これは好酸球に透過電顕的異常があることと、一部の症例で芽球が好酸球に分化したことより推測される。第2群は、反応性の好酸球増多である。今回の検討では患者血清中のサイトカイン濃度には明らかな異常は見いだせなかったが、サイトカイン異常により引き起こされたと思われる間質性肺炎や膠原病様症状を呈したMDS-EO患者を認めたことより推測される。

Objective To report the presence of chromosomal abnormalities in 7% of patients with MDS (MDS-EO). To understand the mechanism of the increase of the disease, and to try to determine the accuracy of the disease group. Results: (1) 8 cases of MDS-EO patients were diagnosed as having high bone mass and high acid content. In 3 cases, the morphological abnormalities of granules and the maturation of cytoplasm were recognized. (2) In MDS-EO patients, the growth of endosperm and buds was recognized in 3 cases, and the buds were extracted from the cell sorter and cultured in the presence of various media for a short time. In the presence of IL-3,IL-5 and GM-CSF, the differentiation of the buds in 1 case was good. This is the first time I've ever seen you. (3) Comparison of serum concentrations between MDS patients and MDS-EO patients. IL-1β, IL-2,IL-3,IL-5,IL-6,G-CSF,GM-CSF are not the same. (4)The supernatant concentration of 3 MDS-EO patients after short-term culture was significantly increased. (5) There are both intrinsic and specific chromosomal abnormalities in MDS-EO patients. The etiology of MDS-EO patients was investigated in two groups. The first group is more acid balls. A part of the disease is caused by the abnormal transmission of the acid ball. The second group of anti-acid balls, anti-acid balls increased. In this study, patients with interstitial pneumonia and collagen symptoms were identified.
