目的:過敏性腸症候群(IBS)における心理社会的ストレス負荷時の認知的評価と副腎皮質活動(コルチゾール・DHEA)および自律神経系活動を病型(下痢優位型・便秘優位型)も考慮して検討した.方法:大学生を対象にした(便秘型IBSのサンプル数確保のため性周期を統一した女性も加えた).IBSの診断基準であるローマ基準IIを満たす者をIBS群,IBSを伴わない者を統制群とし,さらにIBS群を下痢優位型,便秘優位型,特定不能型に分類した.なお,本研究では,IBS群の病型別の比較において特定不能型は除外することとした.対象者に対しスピーチ課題および暗算課題を含む実験ストレッサーを負荷し,その後60分間の回復期を設定し,ストレス反応惹起から回復までの各指標の変動を確認した.実験中,主観的ストレス(VAS : 0-1),唾液中のコルチゾールおよびDHEA,自律神経活動を繰り返し測定した.日本語版Stress Appraisal Measure(予期的ストレッサーに対する認知的評価.脅威,コントロール可能性,挑戦の3因子)は課題直前に,自覚的ストレス尺度(PSS:最近1週間のストレス)は実験直前に記入を求めた.コルチゾールとDHEAの底辺を基準にした曲線下の領域を計算し,コルチゾールとDHEAの反応性を評価した.結果:対象者62名(男性51名,女性11名)中,15名がIBS群となった.IBS群のうち下痢優位型が7名、便秘優位型が2名、特定不能型が6名であった.病型に分けずに検討することによっても新たな知見を示すことができると考えられるが,病型による比較を行うことで,心理的介入において病型によって工夫が必要か否かを検討することが可能となる.今後も病型による比較が可能となるよう対象者を追加する予定である.

Objective: To evaluate the cognition of irritable bowel syndrome (IBS) during psychosocial stress and the activity of adrenal cortex (DHEA) and autonomic nervous system (DHEA).
