妊娠中や産後に尿失禁を経験する女性は多く、関連する諸要因の一つに妊娠末期から分娩第１期に骨盤内を下降する児頭による膀胱への圧迫侵襲がある。しかし、児頭下降による膀胱への圧迫侵襲状態については、これまで視覚的診断方法である画像から膀胱の形状変化を解明した研究はない。本研究では、経会陰超音波断層法検査を用いて画像診断し、膀胱の形状変化と尿失禁症状との関連を検証することを目的としている。妊娠36週以降の単胎妊婦14名を対象とし、妊娠36週以降の妊婦健診時に経会陰超音波断層法検査を行った。尿貯留量を統一するため、測定は最終排尿後60～90分に行い、測定体位は同一内診台上で砕石位とした。プローブの縦軸を陰唇の縦方向に接触させ、恥骨・膀胱・尿道・児頭・腟を含む矢状断面を描出した。恥骨の長軸をランドマークとし、尿道を描出できる程度の圧迫をメルクマークとした。恥骨最大径を結んだ直線と恥骨下縁を起点として膀胱下端までの直線がなす角度を膀胱下降度と定義し、その角度を測定した。本研究課題は、滋賀医科大学倫理審査委員会の承認を得て実施した（整理番号R2021-151）。経会陰超音波断層法検査画像から、妊娠末期の女性の膀胱は、膀胱下端が恥骨下縁を越えて足方に伸展し、うすく平らに引き伸ばされ形状変化している特徴的画像を明確化することができた。また、妊娠末期の女性の膀胱下降度の平均は110.8±9.5°であり、鋭角である非妊時と比べて角度は大きいことが明らかとなっている。
