口腔腫瘍術後の形態変化による摂食・嚥下機能の低下は,食事内容の制限や食べる楽しみの欠如といったQOLの著しい低下を招くほか、特に広範な口腔外科手術では,誤嚥による肺炎や窒息によって死に至る危険性があり深刻である。これまで口腔外科医や麻酔医は術直後から機能の回復に専念してきた。そこで,本企画調査は,口腔腫瘍術後の摂食・嚥下機能障害を1.診断、2.治療、3.予防の3つの観点から徹底した調査をすすめ、口腔腫瘍術後の摂食・嚥下機能に対する効果的かつ包括的な対応策を検討した。1.診断(1) 摂食・嚥下機能障害の病態解明口腔外科術後の摂食・嚥下機能低下は,複数の原因が重なっていることが多い。そこで摂食・嚥下機能障害の機序を解明し,診断方法の確立を行うための症例検討会を実施した。(2) ベッドサイドで使用できる摂食・嚥下機能の総合的評価システムの開発ベッドサイドで口腔腫瘍術後の摂食・嚥下機能障害を正確に診断し,治療経過およびリハビリテーションで経時的に評価する装置を開発するための意見交換を実施した。2.治療(1) 摂食・嚥下リハビリテーションの確立口腔腫瘍術後の摂食・嚥下機能回復を目的に,有効な訓練方法の確立と摂食・嚥下訓練食および嚥下補助食の開発を行うための摂食嚥下リハビリプログラムを作成した。(2) 摂食・嚥下機能回復頸部郭清を伴う口腔外科術後では,副神経保存症例に置いても頚肩腕症候群を誘発することが多く,長期に摂食・嚥下機能障害を術後早期のリハビリが検討されている。過去の症例について頚肩腕症候群のレトロスペクトルな調査を基に対策方法を検討した。3.予防(1) 摂食・嚥下機能を保持する口腔腫瘍手術法に関して検討を行った。(2) 口腔腫瘍術後の頚肩腕症候群予防リハ法の検討を行った.(3) 口腔外科,歯科麻酔科,内科,リハビリ科の連携した介入方法を検討するため関係者会議を開催した.

The changes in oral cavity after surgery are mainly due to the decrease in swallowing function, the restriction of food content, the decrease in QOL, the decrease in oral surgery, the decrease in pneumonia and asphyxia, and the increase in risk. The oral surgeon and anesthesiologist are responsible for the recovery of function after surgery. This study includes: 1. diagnosis, 2. treatment, 3. prevention of oral edema postoperative "eating and swallowing function impairment"; 3. thorough investigation of oral edema postoperative "eating and swallowing function impairment"; and 3. discussion of oral edema postoperative "eating and swallowing function impairment". 1. Diagnosis (1)"Eating and swallowing dysfunction" Pathological explanation of oral surgery "Eating and swallowing dysfunction", multiple reasons are more important. The mechanism of food and swallowing dysfunction was clarified, and the diagnosis method was established. (2)The development of a comprehensive evaluation system for the use of oral cavity function after oral cavity surgery, the correct diagnosis of oral cavity dysfunction, and the implementation of an exchange of views on the development of an evaluation system for the treatment of oral cavity dysfunction. 2.
