【BACKGROUND & PURPOSE】 The purpose of this study was to clear the psycho-socio-behavioral problems of hospitalized adolescents with chronic illness reported by the nurses. In addition with the survey, case conferences with nurses were held to describe such psycho-socio-behavioral problems among adolescents and to find out the direction of nursing care. 【METHOD 1 : Survey】 Self-report and multiple choice questionnaire were sent to 355 hospitals in Japan. Nurses were asked to describe psycho-socio-behavioral problems of chronically adolescents and the process of nursing care for patients and their families. 【METHOD 2 : Case conference】 Case conference were held 6 times and 9 cases were discussed among nurses and school teachers from 4 hospitals and 2 educational institutions. The contents of discussions were recorded and contents were analyzed. 【RESULTS】 The results were as follows : 1) 94 cases with psycho-socio-behavioral problems were reported; 2) characteristics of such issues were different from those of adolescents without chronic illness ; 3) most of issues were induced by physical and emotional burdens of illness, although issues in some cases were present regardless illness ; and 4) nurses regard the importance of family's roles in the process of decision-making. 【PRODUCTS】 Nursing guideline were made and proposed followings : 1) the risk assessment of psycho-socio-behavioral problems among chronically ill adolescents based on the most frequent demographic and illness data of 94 cases ; 2) the nursing assessment process to clear such problems divided by 8 steps ; 3) the directions of new adolescent nursing in terms of the process of being mature decision-makers, maintain self-care and health promotion, and the collaboration with teachers, co-medial staffs, and other institutions.

[背景与目的]了解护士报告的慢性病住院青少年的心理社会行为问题。并与护理人员进行个案讨论，以了解青少年的心理社会行为问题，探讨护理工作的方向。[方法1：调查]向日本355家医院发送自我报告和多项选择问卷。要求护士描述慢性病青少年的心理社会行为问题以及对患者及其家属的护理过程。【方法二：病例会议] 4家医院和2家教育机构的护士和学校教师召开了6次病例会议，讨论了9例病例。记录讨论内容并进行分析。[结果] 94例青少年存在心理社会行为问题，其特点与非慢性病青少年不同，多数问题是由疾病引起的生理和情感负担引起的，但也有部分问题是非疾病性的;（4）护士重视家庭在护理决策过程中的作用。[产品]根据94例慢性病青少年最常见的人口学资料和疾病资料，制定并提出了慢性病青少年心理社会行为问题的危险性评估方法和8个步骤的心理社会行为问题的护理评估流程;（3）青少年护理新模式的方向，包括青少年护理的成熟决策过程、自我护理和健康促进、与教师、医护人员的合作，等机构
