The nationwide biobank for sera and plasma samples from polytrauma patients was initiated in 2013 by the task force Network Trauma Research (Netzwerk Traumaforschung, NTF) (Relja et al. 2020; Huber-Lang et al. 2011). This quality control project was introduced to systemically evaluate and monitor polytraumatized patients in terms of their (patho-) physiological make-up and clinical treatment strategies in order to improve the same. The feasibility of the multi-center NTF Biobank as well as the online tool have been successfully proven and established in seven trauma centers during the pilot phase of five years, which ended in 2018. In the next funding period, the establishment of an NTF-EV-Biobank will be conducted. The objective of this project is to expand the NTF Biobank with NTF-EV-Biobank and provide isolation, storage and management protocols of serum-/plasma- EVs from trauma patients. This includes comparisons of methods for efficient EV isolation, extension of the ethics and software basis to the additional isolation and storage of EVs. Consequently, the first NTF-EV-Biobank pilot study will be aimed to validate the feasibility of the developed EV Biobank and will be focused on comparison of EVs populations in serum/plasma of patients with different injury patterns, collected at different time points after trauma. Therefore, EVs samples from admission to the hospital will be collected according to the new NTF-EV-Biobank isolation and characterization protocol. The NTF-EV-Biobank material will be used to trace tissue-cell-originated EVs by given exosomal cell surface receptors. The surface receptor EV barcode could mirror the injury pattern and therefore could be a diagnostic tool in future. In addition, the EVs from the newly established NTF-EV-Biobank will be used to analyse the EVs cargo. Especially, the short half-life inflammatory markers are hypothesized to be preserved from degradation in EVs and therefore could be used as a diagnostic and prognostic markers in polytrauma patients
