项目摘要

HLA-A24及びHIA-A2陽性高度進行癌患者を対象とし,われわれが保有するペプチド分子に対する癌患者末梢血リンパ球中のCTL前駆体の有無をCTLマイクロアレイ法にて同定し、CTL前駆体が確認されたペプチドを最大4種類投与するテーラーメイド型ワクチン療法の第1相試験を計画し、その有害事象の種類や程度と免疫反応性(CTL誘導能とペプチド特異的IgG抗体誘導能)の解析と共に、ワクチン継続投与症例においては、探索的研究として抗腫瘍効果についても検討し、第II相試験に移行するための適切なプロトコール作成を目的として研究を実施した。本学倫理委員会で承認を得た後に第I相試験を実施した。使用ペプチドは、HLA-A24患者用が14種類、HLA-A2患者用が16種類で、対象疾患は肺、大腸、婦人科、前立腺、メラノーマ、胃及びその他の癌で、各々の登録例数は10例とした。ペプチド投与量は3mg/bodyに設定し、adjuvantとエマルジョン化して皮下に2週毎投与した。後治療は、免疫反応性を認め患者が希望した場合に限り継続し、その臨床効果も検討した。登録例数は、HLA-A24陽性が56例、HLA-A2陽性が28例で、登録終了はHLA-A24の肺癌、大腸癌及びHLA-A24/A2陽性の前立腺癌であった。登録終了HLA-A24陽性例の有害事象をみると、注射部の局所反応を主体に種々の有害反応がみられたが,いずれもGrade 1-2であった。免疫反応性では、27例中14例にCTL増大を認め、抗体発現は12例に認めた。抗腫瘍効果は、PR3例、SD7例、PD22例であった。また、後治療時の化学療法との併用でPR2例、MR2例を認めた。以上の結果より、肺癌、大腸癌、前立腺癌に対して早期第II相試験のプロトコールを作成し、倫理委員会の承認を得て現在臨床試験を実施中である。

HLA-A24 and HIA-A2 positive patients with highly advanced cancer were targeted for the presence or absence of CTL precursors in the peripheral blood of cancer patients. CTL screening method was used to determine the presence or absence of CTL precursors in the peripheral blood of cancer patients. The analysis and analysis of the types and degrees of harmful events and immune responses (CTL inductivity and IgG antibody inductivity), the study of anti-tumor effects, the study of phase II migration, the study of appropriate anti-tumor effects, and the study of clinical symptoms. The Ethics Committee of the University acknowledges that the first phase of the trial has been carried out. 14 types of HLA-A24, 16 types of HLA-A2, 10 cases of lung, colon, female, prostate, stomach and other cancers were registered. The dosage is set at 3mg/body and administered subcutaneously for 2 weeks. After treatment, immune response, patient's hope, clinical results, etc. The number of registered cases was 56 HLA-A24 positive cases, 28 HLA-A2 positive cases, lung cancer, colorectal cancer, and advanced adenocarcinoma HLA-A24/A2 positive cases. HLA-A24 positive cases were identified as HLA-A24 positive cases, and HLA-A24 positive cases were identified as HLA-A24 positive cases. The immune response was positive in 14 out of 27 cases, and the CTL was increased in 12 out of 27 cases. Anti-tumor effect: PR3 cases, SD7 cases, PD22 cases 2 cases of PR and 2 cases of MR were identified. The results of the study include lung cancer, colorectal cancer, and prostate cancer, and the establishment of an early Phase II trial, and the approval of the Ethics Committee, and the implementation of the current clinical trial.
