项目摘要

【目的】第２次がん対策推進基本計画では平成34年までに成人喫煙率を20％から12％に低減する目標を掲げている。医療機関や行政は啓発活動や禁煙支援を行っているが、いまだ十分とは言い難い。また保険薬局では、禁煙指導が調剤報酬に算定されないことや調剤・服薬指導業務のため十分な禁煙支援が行えないとの声も多い。本研究では保険薬局で行える簡便な禁煙支援法を開発することを目的とし同法の効果を検討した。【方法】同意を得た長崎市薬剤師会会員薬局の18薬局で研究を行った。18薬局を無作為に２群（介入薬局群と非介入薬局群）に分けた。対象者は６ヵ月以上継続して来局している喫煙者の中で研究に同意を得た者とした。介入薬局群に来局した対象者へは通常の服薬指導と共に事前に作成した禁煙啓発リーフレットを投薬時に調剤済薬剤へ半年間同封した。非介入薬局群では来局した対象者へ従来通りの投薬と服薬指導を行った。両群とも患者の意志による禁煙相談については対応を行った。研究開始時と研究開始６ヵ月後の来局時において質問紙法と禁煙行動の有無を調査した。又、禁煙行動が認められたものについては、さらに６ヵ月後の禁煙継続について追跡した。【結果】対象者は231名（介入薬局群：146名、非介入薬局群：85名）であった。うち研究開始６ヵ月後に有効な回答を得られた124名（介入薬局群：74名、非介入薬局群：50名）について検討した。禁煙試行者は介入薬局群９名、非介入薬局群１名であり、本研究で開発した禁煙支援法により禁煙試行率が有意に上昇した（p=0.048）。禁煙を試行した者でさらに６ヵ月間の禁煙を継続した者は介入薬局群３名、　非介入薬局群０名であり有意差はなかった。
