【目的】福島原発事故による避難生活で在宅介護する介護者を対象に、精神的健康と介護負担感について1年後の変化を追跡し介護者への支援について検討した。【方法】初回調査(2016)に協力が得られた対象者に調査を依頼し、追跡調査に協力が得られた対象者へ訪問による聞き取り調査を実施した。調査内容は、被介護者の基本属性、介護保険制度の利用状況、認知症等の状態、介護者の基本属性、精神的健康（改訂出来事インパクト尺度日本語版：IES-R、日本語版General Health Questionnaire：GHQ12）、介護負担感（Zarit介護負担尺度日本語版）とした。分析は主な変数について初回調査時と追跡時の変化について検討した。【結果】被介護者の状況に変化はなかった。介護者の精神的健康は、初回調査時と追跡時との変化として、IES-R平均値は有意に低下（p=.002）していたが、合計得点25点をカットオフ値とした場合のPTSD疑いがある者は減少したものの有意差はなかった。また、GHQにおいても変化がなく、介護負担感は減少したものの有意差はなかった。【考察】避難指示が解除される時期となり、介護者や被介護者においては、今後の生活基盤の再構築を余儀なくされ、度重なる生活の変化による心理的な影響や介護を担うことによる健康への影響が懸念された。【結論】避難生活5年目で在宅介護をしている介護者の精神的健康および介護負担感は1年前と同程度であった。長期避難生活で在宅介護を継続している介護者に対して、精神的健康に対する支援とともに、介護負担軽減のための支援の必要性が示唆された。

[Objective] To investigate the changes in mental health and the burden of care after the Fukushima accident, including the impact of the evacuation on the life of the caretaker in the home, the follow-up to the caretaker's support, and the evaluation of the mental health and the burden of care. Analysis of the main number of initial callback time and trace time of change in the investigation [Results] The status of the person being protected changed. The mental health of the mediator is lower than that of the initial correction (p=.002), and the average value of the IES-R is lower than that of the initial correction (p=.002). In addition, GHQ has been working hard to reduce the burden of mediation. [Investigation] Evacuation instructions are released during the period, the mediator and the protected person, and the future life foundation is reconstructed. The psychological influence and the health influence of the mediator are suspense. [Conclusion] The mental health and burden of caregivers in the 5-year period of refuge life were the same as those in the previous year. The need for long-term shelter support for mental health and the reduction of the burden of care is demonstrated.
