项目摘要

高齢者の医療と介護の連携を推進するために、近年様々な法整備がなされているが、高齢者の生活を支えるシステムを構築するうえで、その連携エリアをどのような単位で考えるかは重要である。このような視点に立ち、今年度行った研究は以下の通りである。(1) 第8期介護保険事業計画及び第7次医療計画における地域包括ケアシステムの進捗度調査（計画）第8期介護保険事業計画（2021年～2023年度）及び第7次医療計画（2018年度～2023年度）にて推進する地域包括ケアシステムについて、各自治体の進捗状況を把握するため、調査準備を行った。すでに本研究で実施済みの2019年調査で、（1）在宅医療・介護連携の推進、（2）介護サービスの体制整備、（3）介護予防サービスの体制整備、（4）生活支援サービスの体制整備、（5）住み続けられる高齢者住まいの整備、（6）地域ケア会議の推進、（7）地域共生社会の実現の各項目に関する進捗度の回答を得ており、約５年後の状況を比較するものである。年度当初は2022年度に調査を実施する予定であったが、コロナ禍の終息が見込めず、調査先の負担を考慮し、2023年度の実施へ見送ることとした。医療と介護の連携エリアの設定状況と、地域包括ケアシステムの進捗状況の関係性を分析する予定である。(2) 高齢者の生活支援に関する調査徳島県南部の漁村集落において、高齢者の生活支援を行う団体に、支援内容、対象エリア、地域包括支援センター等との連携状況に関する聞き取り調査を行った。対象とした地域は2006年に二町が合併した自治体で、団体が活動するエリアには本庁が無く、以前は地域包括支援センターとの連携もできていなかった。しかし、2020年から高齢者の生活支援を積極的に実施し始めたところ、現在は地域包括との情報共有も進展しているとのことであった。

The relationship between the setting status of medical and nursing care connectivity and the progress status of regions, including ecosystems, will be analyzed and determined. (2)Survey of the fishing village cluster in southern Tokushima Prefecture, survey of the living support of Takayama Prefecture, survey of the connection status of the organization, support content, target area, including support center, etc. In 2006, the two cities merged and the two cities merged.
