地域包括支援センターの整備について把握するため，都道府県ウェブサイトからの情報ならびにいくつかの自治体での聞き取り調査等を行った。2006年度から制度化された地域包括支援センターは，おおむね中学校区を念頭においた日常生活圏域ごとに国が示す高齢者人口に応じた3専門職（保健師，社会福祉士，主任ケアマネ）の配置を旨として整備していくこととなっており，高齢者人口が一定数以上の規模で，かつ増加傾向にある地域ほど，その体制整備を増強していく必要性が高い。その際，拠点数を変えずに1つの地域包括支援センターにおける職員配置数を，委託先事業者（社会福祉法人や医療法人等）に対する人件費部分での委託料の増額によって拡充する方法と，圏域を分割再編して地域包括支援センターの数を増やす方法とがある。本研究における調査では，政令市など人口規模が非常に大きな都市部では拠点数を増やすこと以上に配置人員数を増強する方向での対応が目立っている。この背景には，人口規模の大きな都市では，すでに2006年度当初から地域包括支援センターを委託可能な事業者が域内に多く立地しており，政令市などでは市内の全ての法人に委託するまでもなく所定の整備が可能であったため，早い段階で拠点数や圏域の区割りを含めた体制が確立されていた点が指摘できる。これに対して，大都市圏郊外等の人口10万未満程度以上の地域では，高齢者人口の増加に対応するため，圏域再編を伴う新規委託先の獲得による地域包括支援センターの新たな整備を試みる事例が見られる。これら中小規模の自治体では，市直営だった場合を中心に，当初は委託可能な事業者が少なく，相談件数が増加する中で，直営による専門職確保が困難になってきた状況を受けて，地域の事業者への委託に切り替えるケースが見られる。ただし，その際の委託先事業者の確保には，受託辞退など一定の困難さが伴う状況も確認される。
