项目摘要

1)高齢者が寝たきりとなる過程における住居的要因を検討するために, 前年度までに行ってきた事例調査をさらに補足する調査を実施した. その結果, 寝たきり老人形成過程とは, 身体的機能の低下に伴って, 日常生活行為の空間的範囲が極度に狭くなる過程であること, および, その過程で住居的バリヤが大きな影響を与えていることが明らかになった. つまり, 身体の弱まりだけが原因で, 寝たきり老人になるわけではなく, 住居も大きな原因になっている. また, リハビリテーションの不足によって, 寝たきり老人がつくられているが, 住居の中にリハビリを妨げる条件が存在することも確かめられた. さらに, 住居要因は, 本人・家族の寝たきり防止意欲, および介護力の不足と相乗して, 寝たきり老人をつくる原因になっていることも解明された.なお, 住居内の危険要因が, 生活範囲を狭めるバリヤの中で, 極めて大きな役割を果たしていることもわかった.2)住居の改善によって, 寝たきりになるものを防ぐことができるかどうかを検討するため, 寝たきりになりそうな人を対象に, 危険箇所を除去するなどの住居改善を実施した事例(数例)を調査した. 結論を出すには至ってないが, 少なくとも寝たきりになる時期を遅らせる効果のあることが確かめられつつある. このテーマを今後の研究展開の最重点としたい.3)寝たきり老人家庭の事例調査の中で, 寝たきり老人宅には特有の火災危険が存在すること, 避難危険をつくり出している住居的要因・介護要因は, 寝たきりをつくる要因(住居的バリヤ)と共通していることが解明された.4)寝たきりをつくる原因のひとつとしての住居の歩行事故のうち, 従来の研究の遅れている「つまづき事故」に関する実験的研究を行い, 足のつま先の軌跡や事故の発生メカニズムをある程度解明した.
