项目摘要

1.QOL(生の質Quality of Life)という評価の観点は医療の現場で最近注目されつつあるが、この概念に関して混乱も見られる。例えば癌の化学療法の現場では身体的な快適さ、不快さに限定してこれを考えようとする傾向がある一方、患者の全人的状態に注目する際には患者が現に充実した生を送っているかどうかまでをこれに含めて考える立場もある。これについては哲学・倫理学の歴史を踏まえた検討を通して、本研究は「患者の人生の可能性ないし選択の幅がどれほどあるか」ということをQOLの最も一般的な定義とすることによって、問題を整理することを提案する(清水)。2.告知の是非、情報を得た上での同意(informed consent)という論点も、QOLの向上を目指す医療の過程として位置付け、整理すべきである。ことに前者については現行のQOL評価の指標では不十分であって、自己の状況を認識しているかどうかを、もう一つの指標として採用する必要がある。3.〈死〉についての日本語の使い方の分析から、人格の死と身体の死が、既に日常言語において使い分けられていることが指摘出来る。人間の死として決定的なのは人格の死(=コミュニケ-ションの不可逆的断絶)であり、その死の判別基準として身体の死が採用されている、という構造になっている。ここから脳死を死の基準とするかどうかという問題へのアプロ-チも可能であろう。4.研究は分担者同士の検討のほか、他の研究者と意見交換をしつつ進められたが、特に新田は北海道哲学会シンポジウムにしてパタ-ナリズムをめぐり研究成果の一部を論じ、また清水は東札幌病院主催の生命倫理研究会において毎月発表を重ね、さらに平成二年二月開催の「第4回冬季札幌がんセミナ-」にて以上の成果の概要を発表した。
