痴呆患者の意味処理能力の低下を引き起こすメカニズムを検索することを目的に、痴呆患者の意味処理能力の縦断的追跡を行った。1.対象:アルツハイマ-型老年痴呆患者軽度7名、中等度8名(軽度患者の追跡研究見通しのため中等度を含めた)。2.検査:高次脳機能検査老研版(見当識、記憶、言語、視空間認知・構成の各領域を含む)。意味処理に関する課題(1)漢字50単語の読解検査:漢字と絵の照合,(2)単語の意味把握検査:具象・抽象語の意味を説明する。(3)単語のカテゴリ-分類:4つの異なる上位概念に各々属する単語を分類する課題。3.重症度別結果の概要:(1)見当識:軽度、中等度ともに見当識の成績にはバラツキが見られた。(2)記憶障害:軽度では直後再生記憶の障害、中等度では直後再生記憶、長期記憶の両方に障害が認められた。(3)言語、視空間認知・構成:重症度が増すにつれて障害も重度になる。言語と視空間認知・構成との関係を見ると、軽度に比べて、中等度で言語と視空間認知・構成能力の成績に解離の見られる症例数がより多かった。(4)漢字50単語の読解検査:軽度では障害なし。中等度では全員障害が認められた。(5)単語の意味把握検査:軽度では障害なし。中等度では8名中、4名に障害が認められた。(6)単語のカテゴリ-分類:軽度では分類すべきカテゴリ-に含まれなかった語数は全課題数の2%に満たなかったが、中等度では23%に及んだ。また、あるカテゴリ-に含まれるべきでない語がそのカテゴリ-に含まれる語数が、軽度で前課題数の1%、中等度で6%あった。(4)〜(6)の結果から、単語の意味理解は痴呆の重症度が進行するに連れて低下すること、中等度痴呆患者では、単語の上位概念とその下位概念との対応関係の崩壊が認められ、意味記憶構造の障害があること、が示唆された。今後、アルツハイマ-型老年痴呆(軽度)について縦断的に追跡を重ね、意味処理能力の崩壊過程と痴呆の進行過程との対応の確認が必要である。
