1983年MarshallとWarrenによって報告されて以来、Helicobacter pylori(以下HPと略す)と胃疾患との関連性が示唆されている。特に、血清中のHP抗体価および胃粘膜組織中のHPの有病率と慢性胃炎や胃癌との関連性が注目されている。本研究は、(1)一般住民におけるHP抗体価の状況および胃粘膜組織中のHPの有病率、(2)各種胃疾患とそれら病理組織とHPの関連、(3)HP抗体価等とライフスタイルとの関連などを知ることが目的である。一般住民のHP抗体価については、秋田県、岩手県、宮城県の赤十字血液センタ-の供血者を対象として、性、年齢(6階級)層化抽出(各40名)合計1,366人の血清についてELISA法(米国クエデル社製キット)で測定した。現時点で結果の判明している宮城県分の454名の測定によるとHP抗体有病率は加齢と共に上昇するが50歳から上昇がにぶる。また、全体として男性の有病率が女性より高いが20歳代、10歳代では逆転し女性の方が高い。このことと関連して、同じ検体1,366件(3県分)の血清ペプシノ-ゲン値も測定した。地域差、胃癌SML、およびHP有病率との関連について分析中である。なお、近く沖縄住民の血清について検索を行うことにしている。胃粘膜組織中のHP有病率については、胃集検で行われた胃生検の組織標本の中から、内視鏡的胃びらんが明らかで、病理組織学的にも胃炎像(大部分は表層性慢性胃炎)を示す112例を対象に、性別、年代別(40歳代と60歳代)、部位別(前庭部と境界部)の8区分毎にメチオニン染色による顕微鏡下の検索を行った。112例中73例(65%)にHPの感染が認められた。男性76%、女性54%、40歳代82%、60歳代45%、部位別では前庭部82%、境界部58%であった。一般住民の血清HP抗体価陽性率との相違がみられ検索対象の偏りなのか、測定法によるものなのか、また、病理組織上の差なのか、今後の検討の必要がある。

Since Marshall and Warren's 1983 report, the relationship between Helicobacter pylori and gastric disease has been shown. In particular, HP antibody levels in serum and HP prevalence in gastric mucosa tissues are closely related to chronic gastritis and gastric cancer. The purpose of this study was to: (1) understand the status of HP antibody in the general population and the prevalence of HP in gastric mucosa tissues;(2) explore the relationship between HP and various gastric diseases; and (3) explore the relationship between HP antibody and gastric mucosa tissues. A total of 1,366 serum samples were collected from the blood donors of the general population (40 from each of the six classes), including Akita Prefecture, Iwate Prefecture and Miyagi Prefecture. The results showed that the prevalence of HP antibody increased from 50 to 50 in Miyagi Prefecture, and the prevalence of HP antibody increased from 50 to 50 in Miyagi Prefecture. The prevalence rate of the disease in males was higher in females than in females at the age of 20, and in females at the age of 10. 1,366 samples (3 points) of serum samples were detected. The incidence of HP in gastric mucosal tissue is related to the central part of the gastric mucosa, the central part of the gastric mucosa, and the endoscopic gastric mucosa. Pathological images of gastritis (most of which are superficial chronic gastritis) are shown in 112 cases. The examination was performed under a microscope in each of the 8 sections of the subject, including gender, age (40 years old and 60 years old), and location (vestibule and boundary). 73 cases (65%) of 112 cases were infected with HP. 76% for males, 54% for females, 82% for 40-year-olds, 45% for 60-year-olds, 82% for vestibule and 58% for boundary. The correlation between serum HP antibody positive rate and detection target, detection method, pathology, and the necessity of further investigation are discussed.
